Job summary
We
are seeking an experienced ANP/ACP to join our team, which operates across 2 sites and provide primary care services to approximately 11,200
patients.
Pay and hours are negotiable (looking for between 24 to 37.5 hrs per week)
Main duties of the job
We are seeking an experienced Advanced Clinical Practitioner (ACP) / Advanced Nurse
Practitioner (ANP) who provides
high-quality, patient-centred care within the general practice setting. Working
autonomously within your professional boundaries, you will be responsible for
the assessment, diagnosis, treatment, and evaluation of patients presenting
with undifferentiated and undiagnosed conditions, as well as those with chronic
illnesses.
You will undertake comprehensive clinical
assessments, take patient histories, and make safe, evidence-based decisions
regarding treatment and care plans. You will exercise advanced clinical
reasoning and professional judgement to deliver effective care, ensuring a high
standard of service that supports both the immediate and long-term needs of the
practice population.
This role requires working both independently and
collaboratively as part of the multidisciplinary team to ensure continuity,
safety, and excellence in patient care within the primary care environment.

Job responsibilities
Working across the 2 sites, you will;
- Undertake consultations
with patients taking a comprehensive patient history, carry out physical
examination and use their expert knowledge and clinical judgement to identify
the potential diagnosis.

Exercise a higher degree of
personal and professional autonomy using advanced clinical practice skills to
include clinical interventions.

Refer patients for
investigations when necessary/appropriate.

Make a final diagnosis,
decide on and carry out treatment, including the prescribing of medicine or
referring to primary or secondary care specialist services within local agreed
pathways and protocols as appropriate.

Ensure that all treatments
and care provided are based on best practice/evidence based care.

Plan and provide skilled
competent care to meet the patients health and social care needs, involving
other members of the healthcare team as appropriate.

Work with
multidisciplinary and multiagency teams in order to ensure patient needs are
met

Asses and evaluate, with
the patient, the effectiveness of the treatment and care provided and make
changes as needed

prescribing skills for benefits of patient care

Take responsibility and
be accountable for managing own workload, risk assessment and management,
acting within professional codes and appropriately

for advice and support.

necessary written records, in line with NMC guidance and local policies,
guidelines and procedures.
