Job summary
We
are seeking an experienced ANP/ACP to join our team. We
operate out of two sites and provide primary care services to approximately 11,500
patients. They will provide care for the presenting patient,
including initial history collection, clinical assessment, diagnosis,
treatment, and care evaluation. They will provide safe, clinical decision-making
and professional treatment for patients in general practice.
Hours negotiable, ideally 24-37 hours.
Main duties of the job
The Advanced Clinical Practitioner (ACP) / Advanced Nurse
Practitioner (ANP) is an experienced healthcare professional who provides
high-quality, patient-centred care within the general practice setting. Working
autonomously within their professional boundaries, the ACP is responsible for
the assessment, diagnosis, treatment, and evaluation of patients presenting
with undifferentiated and undiagnosed conditions, as well as those with chronic
illnesses.
The post holder will undertake comprehensive clinical
assessments, take patient histories, and make safe, evidence-based decisions
regarding treatment and care plans. They will exercise advanced clinical
reasoning and professional judgement to deliver effective care, ensuring a high
standard of service that supports both the immediate and long-term needs of the
practice population.
This role requires working both independently and
collaboratively as part of the multidisciplinary team to ensure continuity,
safety, and excellence in patient care within the primary care environment.
If the post-holder is an ANP, they must have a digital badge
for advanced practice.

Church Road Surgery and Tile Cross Surgery operate as a
single GP organisation and are part of Our Health Partnership. The two
surgeries share a patient list, staff, and systems. We are accredited as a Safe
Surgery and Armed Forces Veteran Friendly practice. Our Care Quality Commission inspections rated both sites as Good. We have a friendly team of approximately

Undertake consultations
with patients taking a comprehensive patient history, carry out physical
examination and use their expert knowledge and clinical judgement to identify
the potential diagnosis
Exercise a higher degree of
personal and professional autonomy using advanced clinical practice skills to
include clinical interventions
Refer patients for
investigations when necessary/appropriate
Make a final diagnosis,
decide on and carry out treatment, including the prescribing of medicine or
referring to primary or secondary care specialist services within local agreed
pathways and protocols as appropriate
Ensure that all treatments
and care provided are based on best practice/evidence based care
Plan and provide skilled
competent care to meet the patients health and social care needs, involving
other members of the healthcare team as appropriate
Work with
multidisciplinary and multiagency teams in order to ensure patient needs are
met
Asses and evaluate, with
the patient, the effectiveness of the treatment and care provided and make
changes as needed
Use independent/supplementary
prescribing skills for benefits of patient care
Work independently and as
part of a team
Taking responsibility and
being accountable for managing own workload, risk assessment and management,
acting within professional codes and appropriately to the clinical team leader
for advice and support
Undertaking other duties
when required, such as audits or administrative tasks
Maintain contemporaneous
records of the consultation and action using the computer software and where
necessary written records, in line with NMC guidance and local policies,
guidelines and procedures
