Job summary

We are looking to recruit an enthusiastic, motivated, experienced Practice Nurse to join our friendly, dynamic nursing team.

This is an exciting opportunity for the successful candidate to develop their individual skills and experience.

Our nursing team consists of 1 Nurse Team Leader, 3 Nurse Practitioners, 1 Diabetic Specialist Nurse, 3 Practice Nurses, 2 HCA and 2 TNA's.

Main duties of the job

To provide and maintain a high standard of nursing care for our patients, as well as providing nursing assistance to the doctors and other members of the primary healthcare team.

The duties will include all tasks normally undertaken by an experienced practice nurse, such as:

Chronic Disease Management, in line with practice protocols, for COPD, Asthma, CHD, Stroke, Hypertension and Diabetes.

Health Screening to include BP checks, height, weight, BMI, urinalysis, smoking status, new patient checks and venepuncture.

Ear care, advising patients about safe ear care and if necessary undertaking ear syringing.

Providing health promotion for healthy eating, exercise, alcohol and smoking cessation advice.

Complex wound management. Providing diagnosis, management and treatment of leg ulcers using Doppler assessment.

Vaccinations and Immunisations for adults and children.

Travel Health Advice, offering a holistic approach to travel health including comprehensive advice for patients prior to travel.

Family Planning and Women'sHealth, providing a comprehensive family planning and sexual health service to patients. Undertaking a programme of cervical cytology.

Applying and maintaining Infection control measures in line with national and local guidance.

Emergency Management, competent in anaphylaxis and resuscitation techniques.

Maintaining up to date, accurate computerised patient / practice records.

Additional roles as agreed, appropriate to the role and training.

We have 10,800 patients and operate from a single site. We are based in Stanley, Co Durham, which is easily accessible from Durham and Newcastle.

We operate using the SystmOne clinical system.

We are a training practice for both medical students, GP trainees and career start nurses and encourage development within the practice.

Duties and responsibilities:

Professional

  • To undertake assessment and manage programmes of care for patients with chronic disease conditions for example Diabetes, IHD, and hypertension within practice protocols and refer when needed.
  • Offer a holistic approach to travel health by providing comprehensive advice to patients prior to travel including; vaccinations and medicines, safe sex, food hygiene, sun protection

  • Offer advice about childhood and adult vaccinations and ensure vaccines are administered under patient group directions
  • Running well person clinics and health promotion programmes, facilitating change by providing holistic assessment and lifestyle advice on diet, smoking, alcohol intake and exercise
  • Provide a comprehensive family planning and sexual health service and utilise relevant practice protocol guidelines for specific client groups.
  • Assessing problems presented opportunistically by patients, dealing with minor illness.
  • Able to document Consultations according to NMC guidelines
