Job summary
Arrival Medical Practice are
looking for a motivated and enthusiastic Practice Nurse to join our friendly,
patient focused practice team. Chronic Disease management skills are imperative
to the role.
The Practice Nurse will be
responsible for a number of clinical areas such as infection prevention and
control, health promotion, chronic disease management, immunisations, cervical
screening, as well as actively supporting the practice management team in the
reviewing and delivery of clinical policy and procedures.
The Practice nurse will also be
responsible for the effective management of patients with long-term conditions
using evidence-based practice including care for elderly and housebound
patients.
Main duties of the job
The main duties of the role are:
- Health promotion and wellbeing
- Implement and evaluate individual treatment plans for chronic disease patients.
- Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on patients' health
- Provide routine nursing care to patients as required in accordance with clinical-based evidence, NICE and the NSF
- Provide wound care (ulcer/Doppler etc.) to patients
- Provide immunisations (childhood, seasonal etc) to patients
- Provide travel medicine services
- Provide Cervical Cytology screening
- Request and process pathology results as required
- Maintain accurate clinical records in conjunction with existing legislation
- Ensure read codes are used effectively and support with achievement of QOF targets
- Chaperone patients where necessary
- Assist GPs with minor surgery when required
- Support the team in dealing with clinical emergencies and safeguarding matters.
- Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)
- Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

Two GP Partners, 2.5 WTE Advanced Care Practitioners providing acute
appointments, a Physician Associate, Clinical Pharmacist, three Practice
Nurses, a Health Care Assistant and two Trainee Nurse Associates.
