Job responsibilities
Support
the planning, delivery, and monitoring of PCN transformation projects aligned
with NHS priorities and local health needs
Coordinate
project meetings, workshops, and events including agenda setting, minute
taking, and follow-up of actions
Monitor
project timelines and task progress, escalating risks or issues where necessary
Maintain
and update project documentation such as plans, reports, risk logs, and
communications
Support
project evaluations by gathering and analysing feedback, usage data, and
outcomes
Stakeholder Engagement
Build
relationships with GP practices, clinical teams, social prescribers, carer
leads, and community partners
Communicate
clearly with internal and external stakeholders to support project awareness
and participation
Assist
in the development of communications and promotional materials for
transformation projects
Ensure
that patient and public involvement is considered in project delivery, where
appropriate
Service Improvement
Support
Identify
areas for service development through data analysis, feedback, or stakeholder
consultation
Work
with team leads to support continuous improvement approaches (e.g. PDSA cycles,
process mapping)
Contribute
to the writing and editing of improvement plans, business cases, and evaluation
reports
Administrative Support
Provide
general administrative support to the PCN team, including document preparation,
scheduling, and report collation
Support
the tracking of key project metrics and reporting requirements to ICS or
funders
Maintain
confidentiality and adhere to all NHS data governance and information sharing
protocols
Confidentiality
In the course of
seeking treatment, patients entrust us with, or allow us to gather, sensitive
information in relation to their health and other matters. They do so in confidence and have the right
to expect that staff will respect their privacy and act appropriately.
In the performance of
the duties outlined in this Job Description, the post holder may have access to
confidential information relating to patients and their carers, Practice staff
and other healthcare workers. They may
also have access to information relating to the Practice as a business
organisation. All such information from
any source is to be regarded as strictly confidential.
Information relating to
patients, carers, colleagues, other healthcare workers or the business of the
Practice may only be divulged to authorised persons in accordance with the
Practice policies and procedures relating to confidentiality and the protection
of personal and sensitive data.

The post holder will assist in promoting and maintaining their own and
others health, safety and security as defined in the Practice Health &
Safety Policy, to include:

according to Practice guidelines.

and undertaking such activities in a way that manages those risks.

maintaining work areas in a tidy and safe way and free from hazards.
