Mid Dorset Primary Care Network
partners Mid Dorset Practices in the delivery of extended primary care
services.The Atrium Health Centre is one of the eight practices that make up Mid Dorset Primary Care Network.
The Practice islooking for an organised, compassionate, and proactive Patient Care Coordinator to join their friendly team.
The hours for this post will be up to 22 hours per week. Days may be flexible by arrangement with the management team prior to employment.
The advertised salary will be pro rata for hours worked.
This role is only open to candidates with UK residential status as we are unable to sponsor applicants at present.
Main duties of the job
As a Patient Care Coordinator, youll be an essential part of administrative team, by supporting the practice team in the management of the treatment recall process and helping to ensure that every patient receives timely, efficient, and considerate support.
Youll work closely with GPs, nurses, and other administrative staff to make sure care pathways are smooth and that patients supported and well informed.
There will also be elements of back office functions in the timely processing of patient information onto the clinical system.

Coordinate
patient appointments for follow up of long term conditions.
Communicate with patients on a daily basis to book appointments in accordance with The Atriums recall processes.
Work closely with the practice team to ensure effective communication and coordination of the patient recall system.
Complete all
mandatory training & maintain a continuing personal development (CPD) plan
Adhere to the
standard operating procedure put in place by the primary care network (PCN)
Adhere to data
protection legislation and data sharing agreements.
Work closely with
and build relationships with key members of GP practice and PCN staff and
attending relevant meetings.
Work as part of the
multi-disciplinary team.
Communicate
politely and effectively with patients, carers, and colleagues.
At all times, to
maintain the highest standards of behaviour, to comply with and follow practice
and CQC policies, protocols, and procedures, including information governance,
health and safety, equality, and diversity and to report any breach or
suspected breach immediately.
To
undertake any other activities that may from time to time be reasonably
requested by the Partners or surgery Management Team.
