Main duties of the job

Key Responsibilities,

  • Deliver high-quality, evidence-based nursing care
  • Undertake diabetes reviews, monitoring, and patient education
  • Support management of long-term conditions (e.g. asthma, COPD, hypertension)
  • Provide routine nursing services including:
    • Immunisations and vaccinations
    • Wound care and dressings
    • Cervical cytology (if trained)
    • Health checks and screening
  • Promote health and wellbeing through lifestyle advice
  • Maintain accurate and contemporaneous clinical records
  • Work collaboratively with GPs and the wider multidisciplinary team

    The Practice Nurse will:

    • Deliver nursing care to patients across a range of clinical areas
    • Support the management of patients with long-term conditions, with a focus on diabetes
    • Promote health and wellbeing through preventative care and education
    • Work both independently and as part of the wider practice team

    Job Responsibilities

    Clinical Care

    • Provide high-quality, evidence-based nursing care in line with NICE guidelines
    • Deliver routine nursing services including:
      • Wound care and dressings
      • Suture and staple removal
      • Cervical cytology
      • Immunisations (routine, childhood, and travel)
      • ECGs and spirometry
    • Undertake new patient health checks and routine screening

    Chronic Disease Management

    • Support the management of patients with long-term conditions, including:
      • Diabetes (essential)
      • Asthma and COPD
      • Hypertension
    • Carry out reviews, monitoring, and patient education
    • Support patients to self-manage their conditions

    Health Promotion & Prevention

    • Deliver opportunistic health promotion and lifestyle advice
    • Support national and local health campaigns (e.g. vaccinations, NHS health checks)
    • Promote patient wellbeing and disease prevention

    Infection Prevention & Control

    • Work in accordance with infection prevention and control policies
    • Support IPC audits and compliance activities as required

    Medicines Management

    • Administer medications in accordance with PGDs and scope of practice
    • Provide advice to patients on medication use, side effects, and compliance

    Patient Management & Administration

    • Request and process pathology tests as appropriate
    • Maintain accurate, clear, and contemporaneous clinical records
    • Refer patients to GPs or other healthcare professionals when appropriate

    Team Working

    • Work collaboratively with GPs, nursing colleagues, and the wider MDT
    • Support colleagues and contribute to a positive team environment
    • Participate in practice meetings and training

    • Diabetes qualification or relevant training/experience
