Wokingham North Primary Care Network based at Woodley Centre Surgery

Care Coordinator Coordination of WNPCN Group Consultation programme.

Part time 18 - 22.5 hours (Wednesday late working required).

Salary Negotiable

Main duties of the job

Wokingham North PCN runs regular group consultations for patients, online and face to face. Topics covered range from diabetes, the menopause and mindfulness and we are looking for a dynamic and committed individual to coordinate the programme.

You will be responsible for all administrative aspects, including coordinating clinician availability, inviting patients, and reviewing feedback.

You will also attend each session as a facilitator, ensuring the session runs to time, coordinating questions and answers and participating in goal planning for patients.

In addition, you will provide support to our Health and Wellbeing Team in their various roles. This is a rewarding job, giving you an opportunity to be part of an innovative NHS team.

The role is based at Woodley Centre Surgery, and you will also be working with staff and patients from Parkside Family Surgery and Wargrave Surgery.

Job responsibilities

JOB DESCRIPTION

Role

Coordinate the running of the WNPCN Virtual and Face to Face Group Consultations and act as facilitator at these events. The Group Consultations are run as one-off events or as a programme of six to eight sessions.

Provide support to the PCN Health and Wellbeing Team.

Main responsibilities

Administration

Arrange planning meeting with all relevant staff for upcoming Group Consultations and agree the agenda.

Coordinate availability of presenters and confirm programme date or dates.

Identify appropriate patients to invite, create and send initial text invite and reminders.

Monitor registrations for the event and send out relevant paperwork to participants.

Create pre-evaluation and feedback surveys and ensure presenter slides are up to date.

Gather and format any clinical data to be presented.

Meet with presenters to ensure smooth running of the event.

Record all activities onto appropriate spreadsheets.

Post event, enter all participants attendance into their medical records and record any outcome measures and actions required.

Monitor progress of outcome measures and actions to ensure they have been completed.

Review and action patient feedback of event and send out slide presentations.

Facilitation

Attend each group consultation, online or in person.

Introduce the event and presenters, check consent and attendance of patients.

Communicate housekeeping rules and encourage completion of pre-evaluation survey.

Coordinate presenters and any slide presentations.

Monitor and coordinate questions and answers within the event.

Facilitate one to one discussions during the event and support and record patient goal planning where relevant.

Encourage completion of feedback survey at the end of the event.

Support the Health and Wellbeing Team

Work closely with the Health and Wellbeing Team to ensure any referrals received by them are appropriate to their role.

Support the team in their role of signposting and coordinating the patients journey through primary, secondary and community care.

Participate in care planning for groups of patients with specific needs, including those with long term conditions.

TRAINING AND DEVELOPMENT

Stay up to date through attendance at any courses and/or study days necessary to ensure that professional development requirements are met, demonstrating skills and activities to others who are undertaking similar work.

Subject to a performance review, including taking responsibility for maintaining a record of own personal and/or professional development.

Work closely with other clinical staff and administrative managers in the setting up and/or improving of practice systems for monitoring/measuring performance against Clinical Governance and Quality Indicator targets

Work to deliver the NHS contract requirements related to the practice (including the terms of the Quality and Outcomes Framework and locally enhanced services)

GOVERNANCE

  • Produce complete and accurate records of patient consultation, in line with best practice, confidentiality, policies and procedures
  • Deliver care according to NHS guidance, NICE guidelines and evidence-based care
  • Take part in the maintenance of quality governance systems and processes across the Practice and its activities.
  • Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required.
  • Work with other clinical teams on improving the quality of healthcare in response to local and national policies and initiatives as appropriate
  • Evaluate patients response to health care provision and the effectiveness of care
  • Support and participate in shared learning across the practice and wider organisation
  • Manage, review and identify learning from patient complaints, clinical incidents and near-miss events
  • Awareness of statutory safeguarding, notification processes and local guidance for children/vulnerable patients, applying relevant policies and legislation to protect them
  • Ensure compliance with policies, procedures and guidelines for self and others, by taking action or alerting senior management team if the practice appears to contravene policy, or if there are concerns over any aspect of patient care.

  • The post-holder will manage their own and others health & safety and infection control as defined in the Practices Health & Safety Policy, the Practice Health & Safety Manual, and the Practices Infection Control Policy and published procedures.

  • Awareness and compliance with national standards of infection control, hygiene, regulatory / contractual / professional requirements, and good practice guidelines.
