Paramedic Practitioner Triage & Urgent Care

Job summary

We are seeking a motivated and forward thinking Paramedic Practitioner to join our expanding urgent care team at Haslemere Health Centre. This role offers an exciting opportunity to lead on the development of a new formal triage model while working closely with our GPs, nurse practitioners and experienced reception team. You will play a central part in strengthening our same day access provision and supporting high quality clinical care across the practice.

Location: Haslemere Health Centre & Fernhurst Branch Surgery

Hours:28 to 37.5 hours per week

Core hours:8am to 6.30pm Mon - Fri excluding bank holidays

Salary: £45,000 - £50,000 FTE (non prescriber); £50,000 - £56,000 FTE (prescriber)

Annual leave: 25 days annual leave, Bank Holidays, up to 2 long service days (all pro rata and adjusted for compressed hours).

Main duties of the job

  • Design and implement a formal triage pathway, including establishing a triage hub, creating workflow processes, introducing a RAG risk stratification system, and developing clear patient pathways
  • Join the duty doctor as part of the same day clinical team to manage urgent on the day presentations.
  • Assess, treat, and appropriately escalate patients presenting with acute illness or injury.
  • Work collaboratively with the duty doctor to ensure safe and efficient patient flow during periods of high demand
  • Conduct home visits for acute presentations when required.
  • Provide high quality autonomous clinical assessment, decision making, and management.
  • Contribute to improving patient experience, reducing delays, and ensuring clinically safe triage outcomes.
  • Actively contribute to refining urgent care pathways and supporting ongoing quality improvement.
  • Work closely with the wider multidisciplinary team to optimise access and ensure patient centred care.

About us

Haslemere Health Centre is a large, friendly, and enthusiastic GP practice set in the beautiful Surrey Hills. Our multidisciplinary team includes GP Partners, Salaried GPs, Nurses, Healthcare Assistants, Pharmacist, Pharmacy Technicians, Physiotherapist, social prescriber, and visiting paramedic.

We are a training practice, regularly hosting GP registrars and FY2 doctors. Two of our GPs also run the local GP registrar training scheme.

Our main site is a modern single storey NHS Health Centre on the grounds of Haslemere Hospital, with 20 consulting rooms, a branch surgery in Fernhurst, and dedicated admin space within the hospital.

We are part of the West of Waverley PCN and Procare GP Federation.

Job responsibilities

The postholder will work as an autonomous Paramedic Practitioner as part of the practices urgent care and same day access team. A core element of the role is leading the development and delivery of a safe and effective clinical triage system for the practice. Working closely with the GPs, Practice Manager and multidisciplinary team, the Paramedic Practitioner will support the efficient management of clinical demand and provide high quality assessment and treatment for patients with acute presentations.

Key Duties and Responsibilities

1. Clinical Triage & Demand Management

-Lead the design, implementation and ongoing development of the practices formal triage system.

- Establish and coordinate a triage hub, including workflow design, standard operating procedures and integration with reception and clinical teams.

- Provide clinical triage for same day / urgent patient requests and allocate care based on acuity and clinical need.

2. Urgent Care & MAC Clinics

-Work alongside the duty doctor to deliver MAC clinics and manage urgent on the day presentations.

- Assess, diagnose, treat and appropriately escalate patients presenting with acute illness or injury.

- Provide accurate safety netting, follow up advice and contribute to maintaining smooth workflow during peak demand.

3. Home Visits

-Undertake urgent home visits for acute presentations as required, providing timely assessment and management within the community setting.

4. Multidisciplinary Team Working

-Work collaboratively with GPs, nurse practitioners, other clinicians and administrative teams to support continuity of care and maximise clinical capacity.

- Participate in case discussions, handovers and clinical meetings as appropriate.

5. Clinical Governance & Service Improvement

-Contribute to the evaluation and continuous improvement of urgent care pathways, triage processes and access systems.

- Engage with practice audits, quality improvement projects and safety initiatives.

6. Professional Responsibilities

-Maintain HCPC registration and meet all continuing professional development (CPD) requirements.

- Work within professional boundaries and scope of practice, seeking senior clinical support where appropriate.

- If qualified, practise as an independent prescriber; if not yet qualified, be willing to undertake prescribing training.
