Job summary
The Care Navigator supports the reception and administrative functions at Colton Mill and The Grange Medical Centre. This full-time role (37 hours per week) covers Monday 08:1518:30, Wednesday 07:4516:00, Thursday 08:1518:30, and Friday 08:1518:30.
As the first point of contact, the Care Navigator welcomes patients, manages enquiries, books appointments, and signposts to services within the practice, Primary Care Network, and local community.
Key duties include front desk support, telephone and online request management, maintaining patient records, assisting with prescriptions and clinical administration, and coordinating with clinical staff and external services.
The role requires maintaining confidentiality, data security, and infection control, ensuring the reception is organised and welcoming. Strong communication, organisational skills, and the ability to work independently and as part of a team are essential in this busy healthcare environment.
Main duties of the job

Colton Mill and The Grange Medical Centre is an NHS general practice serving the local Leeds community across two sites.
