Job summary

Are you passionate about medicines management and improving patient outcomes? As our Pharmacy Lead, you'll be at the heart of our Practice, guiding and mentoring the Pharmacy team while sharing your expertise to support GPs and the wider practice. You'll lead on best-practice prescribing, audits, and quality improvement initiatives, helping to shape and develop the pharmacy function to deliver optimal care for every patient.

Main duties of the job

  • Medicines reconciliation and repeat prescription management, implementing changes in line with local and national guidance and responding to medicine shortages.
  • Undertaking medication reviews, including Structured Medication Reviews, face to face, by telephone and remotely, ensuring safe and effective care for patients with chronic conditions. Proactively managing complex polypharmacy, particularly in the elderly, care home residents, patients with multiple co-morbidities, and those with learning disabilities or autism.
  • Clinically assess and treat patients using expert knowledge of medicines for specific disease areas and support chronic disease management through both direct and remote consultations.
  • Provide expert advice to colleagues on prescribing issues, new guidelines, shared care protocols, supply problems, restrictions and contraindications, promoting safe, evidence-based and cost-effective prescribing.
  • Support medicines management within our managed care home.
  • Support and, where appropriate, lead on QOF, enhanced services, IIF and local prescribing quality schemes.
  • Identifies and implement improvements in medication safety, monitoring and prescribing processes, and lead the development and review of internal medicine policies.
  • Liaise with community pharmacies, work closely with and support the Dispensary Lead.
  • Manages, mentor and develop the Clinical Pharmacy team while driving the ongoing growth of the Clinical Pharmacy service within the practice.
