Morley and District Primary Care Network have an exciting opportunity
for a Health and Wellbeing Coach to join the team.
This is a new and important role that will help shape and form our local healthcare offer in Morley, Leeds.
The suitable candidate should be passionate about making a difference in
primary care and enjoy working as part of a multi-disciplinary team across
services.
The role will involve working across the 6 practices we support. so the
candidate must be able to be flexible and adapt to different businesses
These include:
Windsor House Group Practice, Fountain Medical Centre, Leigh View
Medical Practice, South Queen Street Medical Centre, Gildersome health centre
or Drighlington Medical Centre.
Main duties of the job
Health and wellbeing
coaches are non-clinical, personalised care roles. They work with people with physical
and/or mental health conditions, people with long-term conditions and those at
risk of developing them. They focus on improving health related outcomes by
working with people to set personalised goals. Health and wellbeing coaches
offer people support to increase their self-efficacy, motivation, and
commitment to make changes to their lifestyle and improve their health. Health
coaches are experts in behaviour change, working in a person-centred approach.
The expectation of the role is to assist patients either face to face or on the phone (patient dependant) this will involve assessing the patient in a holistic approach to manage their long term conditions through motivational coaching and behaviour change management.

goals, and support them to implement their personalised health and care plan. You willProvide
personalised support to individuals, their families, and carers to ensure that
they are active participants in their own healthcare; empowering them to take
more control in manging their own health and physical wellbeing, to live
independently, and improve their health outcomes through joint care planning.

encourages proactive prevention of new and existing illnesses. They will take
an approach that is non-judgemental, based on strong communication and
negotiation skill, which supports personal choice and positive risk taking,
addresses potential consequences and ensures patients understand the
accountability of their own decisions, making the patient at the centre of
every conversation

- NVQ Level 3, Advanced level or equivalent qualifications or working towards
Desirable
- Training in motivational coaching and interviewing or equivalent experience
