Job summary

We have an exciting opportunity in our Primary Care Network we are currently looking to recruit a clinical pharmacist to develop medicines optimisation services within our Primary Care Network. Morley has a well-established network made up of 6 GP practices covering a population of 63k. It is in a vibrant and busy market town 5 miles outside of Leeds.

The role may be working accross any site within our PCN.

Main duties of the job

The successful candidate will have a patient facing role in managing long-term conditions; working directly with patients to assess and treat conditions, as well as promote self-care.

It is essential you are a confident prescriber ideally with experience of Primary care.

The candidate will have excellent written and verbal communication skills, experience of influencing others and conducting clinical medication reviews.

The successful applicant will work as part of a multidisciplinary team to develop and run (for example processes for repeat prescription reauthorisation, management of medicines on transfer of care and systems for safer prescribing). They will work with and be supported by members of the MDT including pharmacists and pharmacy technicians and other clinical and non clinical roles.

You will be supported by 2 clinical directors and a pharmacy lead.

Qualifications

Essential

  • -Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
  • -Mandatory registration with the General Pharmaceutical Council
  • -Holds an independent prescribing qualification

Desirable

  • -Membership Primary Care Pharmacy Association (PCPA)
  • -Membership of the Royal Pharmaceutical Society
  • -Clinical Diploma or equivalent (demonstrate experience)

Experience

Essential

  • -Minimum of two years experience as a pharmacist, demonstrated within a practice portfolio.
  • -Experience and an awareness of the breadth of common acute and long-terms conditions that are likely to be seen in general practice.
  • -Demonstrates ability to integrate general practice with community and hospital pharmacy teams community groups.
  • -Experience of partnership/collaborative working and of building relationships across a variety of organisations.

Desirable

  • -Experience of working in primary care
  • -Experience of working in a GP practice

Knowledge and Skills

Essential

  • - Minimum of 2 years post qualifying experience.
  • - Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities.
  • - An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
  • - Knowledge of IT systems, including ability to use word processing skills, emails and the internet to create simple plans and reports.
  • - Able to plan, manage, monitor and review general medicine optimisation issues in core areas for long term conditions.
  • - Able to gain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers.

  • - In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
