Health and wellbeing coaches are non-clinical, personalised care roles.
They work with people with physical and/or mental health conditions, people with long-term conditions and those at risk of developing them. They focus on improving health related outcomes by working with people to set personalised goals. Health and wellbeing coaches offer people support to increase their self-efficacy, motivation, and commitment to make changes to their lifestyle and improve their health. Health coaches are experts in behaviour change, working in a person-centred way they are not directive or prescriptive and they do not give advice. Evaluations show that health coaching can produce positive effects on adoption of healthy behaviours, improving physical activity, weight management, body mass index (BMI), blood glucose levels (HBA1c) and dietary fat, along with improving patient satisfaction and quality of life.
Health and wellbeing coaches will take an approach that considers the whole person in addressing existing issues and encourages proactive prevention of new and existing illnesses. They will take an approach that is non-judgemental, based on strong communication and negotiation skill, which supports personal choice and positive risk taking, addresses potential consequences and ensures patients understand the accountability of their own decisions, making the patient at the centre of every conversation.
Main duties of the job
Take referrals from the practice team including GPS, Practice Nurses, Social Prescribers, Clinical Pharmacists and Physiotherapists who will have identified patients that meet the criteria for Health and Wellbeing coaches.
Coach and motivate patients through multiple sessions to identify their needs, set goals, and support them to implement and achieve their personalised objectives. This will include sitting in consultation with a patient and providing them with advice, guidance, and a management plan, personalised to their individual needs. It could include dietetics and healthy eating, lifestyle medicine and getting active, safe activities and personal coaching and motivation.
Ensure all interventions and coaching are designed to empower patients to be active participants in their own healthcare, empower them to manage their own health and wellbeing, and live independently.
Work with the broader MDT to maximise the support available to patients, to connect patients to community-based activities which support them to take increased control of their health and wellbeing.
It is vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies
- Work with practices to identify trends and patterns of why patients may frequently attend/ not attend their practice appointments which could be affecting their health and well being.
- Promote Health and Wellbeing as an approach across the PCN.
