Job summary

We are looking to expand our PCN MDT Team to work across Maidenhead Practices in delivering patient-centred care by:

Providing first point of contact clinics for patients presenting with undifferentiated, undiagnosed problems; Supporting the management of patient conditions through offering specialised clinics such as (but not limited to) family planning, baby checks, COPD, asthma, diabetes and anticoagulation appropriate training; Provide health/disease promotion and prevention advice; Working with the wider primary care MDT team and social care networks to develop integrated care and fulfil the requirements of the PCN objectives; Assisting with clinical audits, significant event review and other research and analysis tasks; Participating in duty rotas with face to face and digital consultations as well as home visits.

Main duties of the job

Staff undertaking these roles will:

Participate in all treatment and preventative healthcare services in the Practice through telephone, e-consult face to face and Home Visits.

Provide direct clinical care to patients -perform physical examinations, analyse, diagnose and explain medical problems. Request and interpret results of diagnostic tests and laboratory investigations. Refer patients to physicians, medical specialists and other health professionals as indicated.

Candidates will be keen to develop a specialist area of interest to support the PCN Practices.

Maidenhead Primary Care Network, is a group of 9 GP Practices working together with a range of local providers to offer more personalised and coordinated health and social care to the 70000 patient population of Maidenhead in Berkshire.

Our Physician Associates Team along with Paramedics Teams are embedded in Practices with both practice and PCN Clinical supervision and support.

Job description

Job responsibilities

The Physician Associate will act within their professional boundaries to provide care for the presenting patient from initial history taking and clinical assessment through to the diagnosis, treatment and evaluation of care. They will demonstrate critical thinking in the clinical decision-making process, including assessment and diagnostic skills, leading to the delivery of safe care for all patients. They will work collaboratively with the healthcare team to meet the needs of the patients, supporting the delivery of policy and procedures. The PA will provide a holistic and clinical service, with support from GPs as required, implementing agreed management plans and following approved protocols as appropriate. The PA will be expected on a typical day to carry out some home visits and consult with patients who present on the day and via booked appointments. The PA will also review and act appropriately on blood results and referrals/correspondence regarding their patients.

Mentorship and supervision will be provided by designated senior medical personnel. The level and type of supervision will be dependent on the post holders acquisition of skills and knowledge and determined by the organisations clinical governance arrangements.

Primary Duties and Areas of Responsibility

Participate in all treatment and preventative healthcare services in the Practice as delegated and agreed by the supervising GP.

Provide direct clinical care to patients using established clinical guidelines.

Consult and refer patients to physicians, medical specialists and other health professionals as indicated.

Interview patients, take medical histories, perform physical examinations, analyse, diagnose and explain medical problems during surgery consultations and home visits.

Recommend and explain appropriate diagnostic tests and treatment.

Request and interpret results of laboratory investigations when necessary.

Participate in Practice Docman and workflow processes

Perform specialised diagnostic physical exams and treatment procedures.

Instruct and educate patients in preventative health care.

Conduct telephone consultations which may involve discussing the result and implications of laboratory investigations with patients as well as dealing appropriately with routine and emergency complaint.

Order laboratory tests as required and agreed under supervising doctor(s).

Fully document all aspects of patient care and complete all required paperwork for legal and administrative purposes.

Participate fully in the clinicians duty rota including the provision of home visits when appropriate.

Patients with problems needing referral to secondary care should be discussed with the supervising GP before making such referral.

Ensure evidenced-based care is delivered at the highest standards ensuring delivery of high-quality patient care.

Enhance own performance through Continuous Professional Development, imparting own knowledge and behaviours to meet the needs of the Practice and PCN.

To achieve and demonstrate agreed standards of personal and professional development in order to meet the needs of the service.

To participate in the audit process, evaluation and implementing plans and practice change in order to meet patient need.

Contribute positively to the effectiveness and efficiency of the team and work colleagues.
