Job summary
An exciting opportunity has arisen for an enthusiastic, professional and experienced Practice Nurse to join our Nursing Team at Weston House Practice. The ideal candidate will have experience in long term conditions, specialising in respiratory. Due to retirement, we are looking for a candidate to work up to 37.5 hours per week across our two sites in Cheltenham.
Main duties of the job
Experience in chronic disease management and asthma/COPD is essential together with knowledge in other areas such as CHD, health promotion and routine health screening, smears, contraception, sexual health and childhood immunisations. The post holder will assess, diagnose and review patients who have respiratory conditions providing the appropriate treatment, care planning and self help information as appropriate.

Weston House Practice is based in St Paul's Medical Centre with a branch surgery at Up Hatherley in Cheltenham caring for 19,400 patients across both sites. We are a training practice for GP Registrars and our team has 8 GP Partners, 6 salaried GPs, a Lead Nurse , 4 Practice Nurses and a team of HCAs and phlebotomists. We are supported by an experienced management team, care navigators and administrators. We are part of a PCN based in the St Paul's Medical Centre and we benefit from a multidisciplinary team of clinical pharmacists, first contact practitioners, paramedics and social prescribers. We have a very good team ethic and all work together to ensure high quality care for our patients.

Registered Nurse with experience in Respiratory Long Term Conditions who will work within the nursing team providing care to our patient population.
Depending on qualifications and experience, training may be given to work towards delivering care in the following clinical areas:
- Run chronic disease management clinics in any of the following areas Asthma, Diabetes, Coronary Heart Disease and Contraception.
- Assist in and perform routine tasks related to patient care as directed by GPs
- Removal of Sutures
- New Patient Medicals/Urinalysis
- ECGs
- Asthma/Peak Flow Readings/Spirometry
- Venepuncture
- Hypertension Management
- Routine immunisations/Childhood immunisations
- Chaperoning and assisting patients where appropriate
- Requesting pathology tests, for example urine culture, swabs
- Following agreed clinical protocols with referral to GPs as appropriate
