Job summary
We are seeking a Specialist Diabetes Nurse or Practice Nurse with special interest in Diabetes who will work as part of our friendly team providing care to our patient population.
The position is flexible and part time, 2-3 days per week.
Anticipated start date is 1 May 2025.

Main duties of the job
We are looking for an experienced Nurse who is able to run Diabetes clinics, review and action blood results, manage medication and support patients with lifestyle changes and referrals to external diabetes services.

Abbeyview Surgery serves 9150 patients in rural Lincolnshire.
We are a friendly, hard working and innovative team. We are currently registered as CQC Outstanding Practice and have won a number of national and local awards for our work to support and improve patient care.
The Practice is an active member of South Lincs Rural PCN.
In the past, our innovative team have held patient education events, supported the opening of dementia cafe, attended the local school to teach children, We work as a team, and with our PPG to identify ways to improve services for patients of the Practice.
