We are recruiting for a number of Enhanced Care Coordinator roles as part of our expanding PCN team.

Were looking for a compassionate and proactive Patient Support Case Worker to join our Primary Care Network team. This role provides short term, focused support to people with complex social needs such as housing issues, financial hardship, mental health challenges, or the effects of deprivation. You will work 1to1 with patients for up to 12 weeks, helping them stabilise their situation, build confidence, and connect with the right services.

You will manage your own caseload, carry out structured assessments, develop personalised support plans, and work closely with a wide range of community partners. Safeguarding, risk awareness, and a strong understanding of the realities of deprivation are central to the role. This is a great opportunity for someone who is motivated by reducing health inequalities and making a meaningful difference in the lives of vulnerable people across Oldham.

Main duties of the job

Provide 1to1 short term support to patients with complex social needs, helping them stabilise their situation and access the right services.

Carry out structured assessments, develop personalised support plans, and manage a varied caseload independently.

Identify and escalate safeguarding concerns, maintaining accurate risk assessments and documentation.

Work closely with community organisations, housing teams, mental health services, and voluntary sector partners to coordinate support and ensure warm handovers.

Prioritise patients experiencing deprivation or multiple vulnerabilities, contributing to efforts to reduce health inequalities across Oldham.

Record all patient interactions, track outcomes, and support service evaluation and improvement.

Maintain uptodate knowledge through supervision, reflective practice, and ongoing training.

Oldham Central Primary Care Network brings together 11 GP practices to improve the way health and wellbeing services are delivered across our local communities. By working in partnership with community services, mental health teams, social care and the voluntary sector, were able to provide more joined up, proactive support for our population while patients continue to access their usual GP services as normal.

Job responsibilities

Patient Support & Case Management

Provide 1:1 support to patients facing challenges such as deprivation, housing issues, financial hardship, mental health difficulties or chaotic lifestyles.

Manage a caseload of patients whose needs sit between social prescribing and clinical case management.

Carry out structured assessments to understand needs, risks and goals.

Create personalised support plans with clear, timebound objectives.

Offer support for up to 12 weeks, ensuring planned discharge and appropriate followup signposting.

Encourage independence and reduce longterm reliance on services.

Safeguarding & Risk Management

Attend safeguarding meetings when required.

MultiAgency Coordination

Work closely with community organisations, housing teams, mental health services, drug and alcohol services, and voluntary sector partners.

Coordinate referrals and ensure warm, supported handovers.

Advocate for patients when needed.

Health Inequalities & Population Focus

Prioritise support for patients from deprived backgrounds or with multiple vulnerabilities.

Contribute to PCN initiatives aimed at reducing health inequalities across Oldham.

Data, Reporting & Quality

Record all patient interactions and outcomes using PCN systems.

Monitor progress against KPIs and contribute to service evaluation.

Uphold confidentiality and data protection standards at all times.

Professional Development

Keep knowledge up to date in areas such as safeguarding, traumainformed practice, motivational interviewing and local support services.

Attend supervision, reflective practice sessions and training as required.

Other Duties

Undertake additional reasonable tasks that support the effective running of the PCN and neighbourhood team, within your training and professional boundaries.
