Health and Wellbeing Care Coordinator

Job summary

This is an exciting role within our organisation for a Health and Wellbeing Care Coordinator to deliver health coaching support for our housebound patients. We are looking for a motivated and passionate individual who is a trained in or willing to train as a Health and Wellbeing coach to make a positive impact on our patients lives.

Main duties of the job

  • Complete Cognitive Geriatric Assessments (CGAs) for patients with a frailty score of mild to moderate.
  • Support our patients with their annual reviews, care plans and ensuring all services available that are suitable are offered by patients and educate our patients in the benefits of these services.
  • To provide personalised care and support to our patients through social prescribing and encourage them to be active participants in managing their own health and wellbeing.
  • To work with patient with physical and/or mental health conditions, patients with long term conditions and those at risk of developing long term conditions.
  • To support our housebound patients with their annual review for long term conditions (such as foot checks, taking bp readings and coding them etc) and referring them onto appropriate community services.
  • Work sensitively with people, their families and carers to capture key information, enabling tracking of the impact on their health and wellbeing.
  • Work closely with staff across the practice to ensure that the health and wellbeing coaching referral codes are inputted into clinical systems.
  • Maintain effective records of activity and produce reports as required.

  • Hold a recognised healthcare or coaching qualification (or equivalent relevant experience)
  • Good standard of education with excellent literacy and numeracy skills.
  • Evidence of continuing professional development
  • Must hold a valid and in date UK driving license

  • Ability to demonstrate management of own caseload and prioritisation of case work.
  • Able to interpret, translate and deliver key messages to patient groups.
  • Good practical and conceptual knowledge of healthcare improvement methods and community level services for referral purposes.
  • Experience of successfully establishing working relationships within teams across multiple locations.
  • Ability to collect and record information and data, for record- keeping, monitoring and evaluation.
  • Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email and face to face.
  • Strong organisational skills including planning, prioritising, time management, notes writing and record keeping.
  • Ability to recognise gaps in services and to identify and act on safety concerns.
