Job summary

Thornaby & Barwick Medical Group are looking to welcome an Advanced Clinical Practitioner to our team.We are looking for an experienced healthcare professional, either as a qualified pharmacist or advanced nurse practitioner with the ability to prescribe.

We require:

  • A masters degree or equivalent in the relevant area of expertise; that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competencies
  • For advanced practice pharmacists only:qualified from an approved 18-month training pathway or equivalent and qualified as an independent prescriber
  • For Advanced Clinical Practitioner Nurses only:have either graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centres ePortfolio (supported) Route; both pathways enable eligibility for an Advanced digital badge issued by the Centre, demonstrating recognised educational and experiential preparation in advanced practice

The successful candidate would have developed the skills and knowledge to allow them to take on an expanded role and scope of practice caring for patients.

Main duties of the job

For Advanced Nurse Practitioner:

The successful candidate will practice autonomously, within their speciality and be able to provide patient-centred care. This will encompass the skills of clinical assessment, examination, diagnosis, and treatment within an agreed scope of practice.

Be involved in decision-making and support new ways of working that create efficient and patient focused services.

Support the ongoing development of clinical practice and standards of care within the service, including the development of policies, procedures, protocols, and guidelines in collaboration with multi-disciplinary colleagues.

Administration requirements to deliver safe patient care and be involved in the continuity of care.

Involvement in signposting, triage, and seeing patients.

Demonstrate a high level of expertise, providing advice, education, and support to patients, their families, and their carers.

For the Advanced Pharmacist:

An enthusiastic and forward-thinking clinical pharmacist.

Develop and manage all aspects of medicines across the practice network.

Working with highly skilled multi-disciplinary team you will work in a patient facing role based in both of our practice sites. This is an evolving role with huge scope for development.

Thornaby and Barwick Medical Group is part of BYTES PCN and provide services to patients via a multi-disciplinary team of GPs, Advanced Practitioner, Nurses, HCAs, GPAs, Physician Associate, Pharmacy Technician, MSK, Social Prescribers, Adult and children mental health professionals, alongside competent and efficient admin teams and managers.

We are a training practice and regularly host GP Trainees and hold education meetings to develop shared learning.

The Advanced Clinical Practitioner (ACP) has been assessed and deemed as competent in practice using their expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis, and treatment of patients.

The post holder works at the level of advanced clinical practice and has developed and can evidence the underpinning competencies applicable to the specialty or subject area, i.e., the knowledge, skills, and behaviours appropriate to the healthcare professionals (HCP) setting and job role.

The role provides an opportunity for patients to avoid the need to be referred to a GP, thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment.

An ACP is autonomous in making decisions based on assessment, diagnosis and interpretation of test results and will initiate, evaluate, and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care.

The following are the core responsibilities of the Advanced Clinical Practitioner.

There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels, however core responsibilities include, but are not limited to:

Practice in compliance with their respective code of professional conduct and within their scope of practice, being responsible and accountable for their decisions, actions, and omissions at this level of practice

Demonstrate a critical understanding of their broadened level of responsibility and autonomy and the limits of own competence and professional scope of practice, including when working with complexity, risk, uncertainty, and incomplete information

Act on professional judgement about when to seek help, demonstrating critical reflection on own practice, self-awareness, emotional intelligence, and openness to change

Work in partnership with individuals, families, and carers, using a range of assessment methods as appropriate

Demonstrate effective communication skills, supporting people in making decisions, planning care, or seeking to make a positive change

Use expertise and decision-making skills to inform clinical reasoning

Initiate, evaluate, and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care

Exercise professional judgement to manage risk

Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice

Implement and evaluate individual specialised treatment plans for chronic disease patients

Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects to the patients' health

Be a professional role model for the organisation

Actively engage in peer review to inform own and others practice, formulating and implementing strategies to act on learning and make improvements

Actively seek feedback and involvement from individuals, families, carers, communities, and colleagues in the co-production of service improvements

Critically assess and address own learning needs, negotiating a personal development plan that reflects the breadth of ongoing professional development across the four pillars of advanced clinical practice (clinical practice, leadership and management, education and research).

Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

Maintain accurate clinical records in conjunction with extant legislation and to ensure SNOMED CT codes are used effectively

Critically appraise and synthesise the outcome of relevant research, evaluation and audit, using the results to underpin own practice and to inform that of others

Contribute to practice targets (QOF, etc.), complying with local and regional guidance

Chaperone patients where necessary

Support the team in dealing with clinical emergencies

Support the team with all safeguarding matters in accordance with local and national policies

Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

Deliver opportunistic health promotion where appropriate
