Primary Care Dietitian

Job summary

The PCN is looking for a dietitian to be a part of the Diabetic HUB run from the PCN. The Diabetic HUB consists of nursing staff and a GP with special interests. The dietitian would support the HUB, integrating with the team by completing dietic reviews for patients who have had recent medication changes or interventions. The dietitian would also work closely with the pharmacist team to carry out medicine optimisation.

  • Develop weight management groups - which may be run on an ongoing basis by the postholder or a Therapy Assistant with appropriate supervision by the postholder.
  • Scope the possibility of group education for patients with Diabetes
  • Work with the catering team to meet the nutritional needs of men on special diets
  • Write, monitor and update relevant processes, pathways and guidelines
  • Train other members of the healthcare team as necessary.
  • Be involved in service improvement
  • Have the opportunity to engage in research

About us

Southend East PCN is working towards being an Integrated Neighbourhood Team - working with lots of different professionals and organisations to work towards NHS Englands Framework.

The PCN has its own building, situated in Shoebury, with a strong team made up of many different professionals. The PCN works closely with its member practices who always help when needed, and support the PCN in every way they can.
