Main duties of the job
We are seeking
apart-time Clinical Pharmacistwith a preferred interest in diabetes
to join our dynamic team.
Diabetes
Interest Preferred: A specialist interest in managing diabetes is desirable for
this role.
Patient-facing
role focusing on chronic disease management, particularly diabetes and
polypharmacy.
Lead on
clinical medication reviews and proactively manage patients with complex
medication needs.
Provide
expertise in medicines optimisation, quality improvement, and safe prescribing.
Support
staff with prescription and medication queries, acute prescription management,
and medicines reconciliation.
Contribute
to the Quality and Outcomes Framework (QOF), enhanced services, and local
medicines optimisation strategy.

South Oxhey Surgery is a located at a walking distance from Carpenders Park
train station. The practice looks after over 4000 patients, uses EMIS clinical
system. Friendly and supportive team. The surgery offers on site car parking. Diverse and welcoming team.

What You
Will Be Doing
Patient-Focused
Tasks:
Lead on the management of
diabetes care within the practice, providing expert guidance and support for
patients with diabetes.
Conduct clinical medication
reviews, particularly for patients with chronic diseases and polypharmacy.
Review chronic disease blood test
results and action necessary medication changes in a timely manner.
Action medication changes and
follow-up care as advised in hospital letters or discharge summaries.
Provide independent advice on
medicines and escalate to other healthcare professionals when necessary.
Attend clinical meetings to
contribute to patient care planning and multidisciplinary discussions.
Operational
Support:
Oversee repeat prescriptions and
manage acute prescription requests.
Ensure safe and efficient
prescribing practices in line with local guidelines and formulary.
Help reduce prescribing-related
workloads for clinical and non-clinical staff.
Quality Improvement:
Develop and implement medicines
management policies.
Support compliance with CQC
standards for safe and effective care.
Contribute to QOF and other local
quality improvement initiatives.
Professional Development:
Maintain accurate clinical
records using appropriate templates and coding.
Stay current with clinical
knowledge through continuous professional development.

- A collaborative team player with excellent communication skills.
- Eager to grow professionally and contribute to the development of innovative, patient-centred solutions.

- Be registered with the General Pharmaceutical Council and a member of the Royal Pharmaceutical Society.
- Ideally you will be a non-medical independent prescriber.
