Job summary

There will only be 1 vacany for PCN Operational Support Officer within iGPc PCN.

Please be aware this is a network role and will be primarly based at the PCN base, Netherley Health Centre with the expectation that the role will sometimes go to it's member practices.

This role is advertised on Oak Vale Medical Centres NHS Jobs platform for the purpose of supporting the recruitment of this role. The role will be employed by iGPc PCN not the practice.

The Operational Support Officer will lead the team of PCN care coordinators and provide direct line management to them. The post holder will be responsible for managing team attendance ensuring there Is adequate staffing levels and policies and procedures are followed.

The role will lead the delivery of the Primary Care Network (PCN) Multi-Disciplinary Team (MDT) who are responsible for managing the care of people registered with practices within a particular PCN. This will involve coordinating the work of healthcare professionals and non-clinical staff including volunteers involved in the care of patients registered at GP practices within the wider PCN population.

Main duties of the job

The post holder will ensure the team is staffed appropriately to deliver the contract requirements, they will deliver workplans for staff and support staff in designing and delivering service requirements. The post holder will work closely with the PCN Manager ensuring to report back to the PCN manager on performance, deliverables, outcomes, challenges, risks and any other relevant business matters.

The remit of this role is broad and evolving and will require attention to detail in the delivery of high-quality workforce management, project management and day-to-day operational support to the network.

The post holder must ensure that patient safety is at the forefront of all service developments and delivery and that all change is focused on improving services for patients.

Leading Care Coordination and Operational Support

Lead the team of Care Coordinators by having oversight of workstreams, being responsible for reporting back on workstreams, progress, outcomes, risks and challenges.

Overall responsibility for ensuring the weekly PCN led MDT meetings (including the weekly virtual Care Home(s) MDT) and the smooth running of integrated care within the team setting.

Coordinate and manage the administrative functions of meetings.

Manage reporting required and associated within the DES specifications for required services.
