Job summary

You will join the dedicated team of healthcare professionals at Middlestown Medical Centre who have an appetite for innovation and have been part of building a successful new primary care model.

You will work as part of our multi-disciplinary team providing personal medical services to the practice patients. This will include long-term condition management, health screening and promotion, immunisations, treatments and dressings.

Main duties of the job

Our Nursing Team offers a range of clinical services including health promotion and long-term condition management, NHS health checks, hypertension management, contraception and sexual health advice, cervical screening, wound management, travel advice, administration of national childhood and adult vaccinations. You will work closely with the multidisciplinary team.

Middlestown Medical Centre has a large multi-disciplinary team looking after over 10,000 patients. We work across 3 sites - our main site is at Middlestown Medical Centre, and our small branch surgeries are just 5 / 10 minutes away at Flockton and Emley.

Our dedicated team all work hard to do the best we possibly can for our patients. We like to use everyone's skills and to play to people's strengths. With a strong emphasis on training and as a training practice, we welcome GP Registrars, Medical Students, Student Nurses and Apprentices. Job Summary

The Practice Nurse will work as part of the primary healthcare team to provide high-quality, patient-centred care to the practice population. Duties include long-term condition management, health screening and promotion, immunisations, wound care, and supporting treatment services.

This role is suitable for both experienced nurses and newly qualified nurses. Support, training, and development opportunities will be provided to help build competence and confidence across all required clinical areas.

About the Nursing Team

The Nursing Team supports a diverse patient population of approximately 10,000 registered patients. Services provided include:

  • Chronic disease management
  • NHS health checks
  • Hypertension management
  • Contraception and sexual health services (including device fitting/removal)
  • Cervical cytology
  • Wound care and ulcer management
  • Travel health advice
  • Childhood and adult immunisations
  • Ear care and phlebotomy

The team works collaboratively within a multidisciplinary environment to deliver safe, effective, and evidence-based care aligned with NHS England and Integrated Care Board (ICB) priorities. There is a strong emphasis on training, mentorship, clinical supervision, and continuous professional development.

Key Roles and Responsibilities

Clinical Care

  • Deliver safe, effective, and compassionate care within scope of practice
  • Manage and support patients with long-term conditions
  • Undertake health screening, assessments, and health promotion activities
  • Administer immunisations and prescribed treatments
  • Provide wound care, including dressings and ulcer management
  • Perform clinical procedures such as ECGs, venepuncture, and cytology (dependent on competence)

  • Maintain clear, professional communication with patients, carers, and colleagues

Clinical Governance & Safety

  • Work within practice policies, protocols, and procedures

Medicines Management
