Job summary
We are delighted to offer an exciting opportunity for a compassionate and motivated Practice Nurse to join our friendly and forward-thinking GP practice for three days per week (0.6 FTE). This is a rewarding role within a supportive primary care team committed to delivering high-quality, patient-centred care to our local community.
As a valued member of our multidisciplinary team, you will play a key role in providing a broad range of nursing services, including chronic disease management, immunisations, cervical screening, health promotion and preventative care. You will have the opportunity to build meaningful relationships with patients while working autonomously and collaboratively with experienced clinicians and administrative colleagues.

Main duties of the job
The Practice Nurse will deliver high-quality nursing care within a busy primary care setting, supporting patients across all age groups. The role includes running nurse-led clinics, supporting the management of long-term conditions such as asthma, diabetes and hypertension, delivering vaccinations and immunisations, undertaking cervical screening, and providing preventative health advice. You will work both independently and collaboratively as part of a multidisciplinary team to ensure safe, effective, and patient-centred care, while contributing to the smooth day-to-day running of clinical services.

- Competent in immunisations and vaccination administration
- Competent in cervical cytology (essential)
- Experience managing long-term conditions, including asthma, diabetes, and hypertension
- Strong communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Commitment to professional development and high standards of patient care
Desirable
- Experience in phlebotomy or formal qualification in phlebotomy
- Experience or qualification in immunisation programmes beyond core training
- Experience with diabetes care or relevant certification
- Experience in wound care management
- Training or experience in chronic disease management clinics
- Previous experience in a community or primary care setting
- Additional professional development courses relevant to nursing practice (e.g., advanced clinical skills, minor illness management)
