We would love you to
have the following skills and experience
Clinical practice
To Assess, implement
and evaluate treatment programmes that promote health and well-being, including
individual treatment plans for patients requiring longer term treatments such
as wound care, asthma, COPD, diabetes and hypertension, cardiovascular disease,
mental health
To work with other
healthcare professionals in monitoring and managing long term conditions, in
line with national and local policies and practice needs
Review medication for
therapeutic effectiveness, appropriate to patient needs and in accordance with
evidence-based practice
Work with patients in
order to support adherence to prescribed treatments; medication reviews
Support patients to
adopt health promotion strategies that promote patients to live healthily, and
apply principles of self-care; such as promoting exercise, improving diet and
stopping risk taking activities such as smoking and excess alcohol intake
Support and manage
health needs of women presenting for family planning and cervical cytology
consultations
Recognise, assess and
possibly refer patients presenting with mental health needs in accordance with
the NSF for Mental Health
Implement and
participate in vaccination and immunisation programmes for both adults and
children
Meet the needs of
patients presenting for opportunistic wound care
Main duties of the job
General Nursing Duties include:
Dietary advice
Blood pressure checks
Urinalysis
Health screening
Dressings
ECG
Phlebotomy
Routine injections
Contraception
Minor injuries
Routine immunisations and vaccinations
Removal of sutures / clips
Baby immunisations
Health surveillance / general health advice
Assisting with minor procedures in the treatment room
Provide general and specific health screenings to the practice patients (within agreed protocols) with referral to other members of the primary and secondary health care team as necessary
Chronic Disease Clinics (Diabetes, COPD, Asthma)
