Main duties of the job

As a Practice Nurse at Steel City General Practice, youll play a vital role in delivering high-quality, proactive care across our three sites. Your duties will include supporting patients with long-term conditions, delivering immunisations, cervical screening, wound care, travel health consultations, and lifestyle advice.

Youll contribute to the achievement of QOF and other practice targets, using a structured, preventative approach to patient care. Working both autonomously and as part of a supportive multidisciplinary team, youll collaborate closely with GPs, HCAs, and other clinicians to provide holistic, patient-centred care.

Youll also have opportunities to lead on clinical areas aligned to your interests and experience. We offer regular clinical supervision, peer support, and ongoing training to support your development in a forward-thinking and inclusive environment.

Job responsibilities

As a Practice Nurse at Steel City General Practice, you will deliver evidence-based nursing care across all age groups, focusing on long-term conditions, health promotion, and preventative services. You will work autonomously within your scope of practice, utilising clinical decision-making skills to assess, plan, implement, and evaluate patient care. You will collaborate with GPs, other practice nurses, and multi-disciplinary colleagues to meet patient needs and provide leadership and mentorship to Health Care Assistants, HCAs, and Clinical Practitioners. A valid UK driving licence is essential to travel between our three sites and to carry out home visits for vaccine programmes and long-term condition reviews.

Key Responsibilities

Long-Term Conditions

Conduct independent clinics for asthma, COPD, diabetes, coronary, hypertension, anticoagulation, and other long-term conditions.

Maintain and monitor QOF registers via SystmOne, and lead on specific QOF domains to ensure maximal achievement.

Proactively identify patients at risk of long-term conditions and initiate appropriate care pathways in line with NICE and ICB guidelines.

Preventative Healthcare and Screening

Undertake opportunistic screening for blood pressure, BMI, smoking status, alcohol use, and other risk factors.

Deliver health promotion interventions, including smoking cessation advice, weight management, physical activity counselling, alcohol reduction support, and self-care education.

Refer patients to services as needed, including spirometry clinics, social prescribing, and structured weight management programmes.

Provide family planning, cervical cytology, travel health advice, and immunisations for adults and children under Patient Group Directions, PGDs.

Clinical Procedures and Emergencies

Perform phlebotomy and simple wound care including dressings, suture, and clip removal.

Administer vaccines, including travel vaccines, ensuring correct storage, stock rotation, and safe disposal.

Recognise and manage clinical emergencies.

Infection Prevention and Control

Comply with infection control policies and local ICB protocols. Liaise with the Infection Control Link within the practice to minimise healthcare-associated infections.

Apply safe handling and disposal of sharps, biological waste, and specimen collection, and manage needlestick incidents.

Communication and Record-Keeping

Use clear, sensitive communication to obtain informed consent and ensure patient understanding, adapting style to different health literacy levels and cultural backgrounds.

Maintain accurate and contemporaneous patient records in SystmOne, upholding GDPR and confidentiality requirements.

Attend practice and educational meetings, and contribute to clinical audits, guideline development, and team case reviews.

Act as a patient advocate when liaising with secondary care, community teams, and social services.

Leadership and Team Development

Mentor and support HCAs, Nurse Associates, and student nurses. Assess competence, provide feedback, and share best practice.

Promote a culture of shared learning by disseminating new policies, guidelines, and evidence-based initiatives to colleagues.

Lead on local guideline development, audit projects, and practice-based commissioning initiatives where required.

Risk Management and Governance
