Job summary

We are looking for an experienced Advanced Clinical Practitioner to join our Neighbourhood Access Service, part of the innovative New Models Programme. You will work within our practices across the NRES Neighbourhood to deliver high-quality, same-day care to our patients.

This role focuses on urgent and reactive presentations, supporting patients with acute illness, minor injuries, and undifferentiated conditions, while liaising with secondary care for complex cases. Working as part of an integrated multidisciplinary team, you will deliver advanced assessments, diagnosis, treatment, and prescribing, ensuring safe, effective, and patient-centred care.

Main duties of the job

  • Provide same-day clinical assessments for patients referred to the Neighbourhood Access Service
  • Manage undifferentiated presentations, including acute minor illness and common primary care conditions
  • Prescribe and review medication in line with local formulary and NICE guidance
  • Deliver evidence-based interventions for common conditions and minor injuries
  • Support and work cohesively with GPs, ANPs and other ACPs in the Same Day Access Hub
  • Provide oversight and mentorship for more junior colleagues
  • Liaise with community teams, safeguarding leads, and social care for continuity of care
  • Ensure accurate documentation and coding in clinical systems (SystmOne/EMIS)
  • Participate in clinical governance, incident reporting, and service audits

  • Contribute to reducing health inequalities in line with Core20PLUS5 priorities
