Job summary
Preston Park Community Primary Care Network is
looking for a full time Paramedic. You
will work closely with clinicians in all GP practices in our PCN and will play
a vital role in increasing capacity for all appointments including home visits.
You will be responsible for the provision of
paramedic services, working as part of the practices multidisciplinary team,
delivering care within your scope of practice.
You will be required to work autonomously in the practice and in the
community (home visits), providing a range of services such as assessment, diagnosis,
treatment, telephone triage and referring patients appropriately, as well as
supporting the management team in the review of clinical policy and procedure.
Main duties of the job

About us
Preston Park PCN is a group of five surgeries - Beaconsfield Medical Practice, The Haven Practice, Preston Park Surgery, Stanford Medical Centre and Warmdene Surgery. We serve approximately 57,000 patients and aim to support and connect with our local community.
Primary Care Networks (PCNs) bring GP practices together with other local services such as community, mental health, social care and the voluntary sector to look after local populations. PCNs will help to join up services at a local level, focusing on the specific needs of these local populations, with patients still accessing routine GP appointments as they do now.

1. To deliver

Perform specialist health checks and reviews
Perform investigatory procedures as required
Undertake the collection of pathological
specimens including intravenous blood samples, swabs, etc.
Perform a domiciliary (home) visit
8. Provide
an alternative model to urgent and same day home visits for the PCN and
undertake clinical audits.
9. Communicate
at all levels across PCNs and other organisations, ensuring an effective,
patient-centred service
10. Communicate
proactively and effectively with all colleagues across the multidisciplinary
team, attending and contributing to meetings as required and accepting
referrals and referring to specialists services where appropriate.
11. Assess,
diagnose, plan, implement and evaluate treatment and or interventions and
care for patients presenting with an undifferentiated diagnosis, and patients
with complex needs within the clinical and domiciliary setting.
12. Understand
and actively take part in the achievement of all contractual markers relating
to the PCN and GP Surgery.
13. Implement
and participate in vaccination programmes as appropriate.

- To be able to effectively assess, plan, treat and refer patients appropriately and evaluate patient care confidentiality
- Can demonstrate a broad and sound knowledge of the conditions affecting adults and children which are most commonly found within the primary care sector,
- To work within the parameters of your skills/knowledge with due adherence to the boundaries of your competencies, according to the NMC Code of Conduct/ Health & Care Professional Council.
- That you have completed the two-year Consolidation of Learning period as newly qualified paramedic
- Have had a further three years experience as a band 6 (or equivalent) paramedic
Desirable
- Have experience of working to protocols/guidelines and patient groups/ patient specific directives
- Have completed a minor injuries and minor illness course at Level 6
- Ability to undertake pos
