Job summary
About the role
We are seeking a highly motivated and skilled Advanced Nurse Practitioner (ANP)or General Practice Paramedic to join our multidisciplinary team. You will provide high-quality, person-centred care, including assessment, diagnosis, treatment, and prescribing for a range of acute conditions supporting our duty team with daily acute care needs, primarily through same-day and urgent care clinics and telephone triage.
You will work both independently and collaboratively, supporting GPs, nurses, and allied health professionals to deliver safe and effective care to our local population.
The role is currently for 8 weeks fixed term but could be open to review of a permanent position.
Main duties of the job
Key responsibilities
Undertake autonomous clinical assessments, diagnostics, and treatment of patients
Provide expert management of minor illness, long term conditions, and urgent care presentations
Prescribe medications safely under your independent prescribing qualification
Maintain accurate and timely patient records using EMIS
Work in partnership with the wider team to enhance patient outcomes and access to care

Job responsibilities
To provide high-quality, patient-focused urgent care services within the general practice setting. The Urgent Care Practitioner (UCP) will assess, diagnose, treat, and manage patients presenting with urgent and unscheduled care needs. Working both independently and as part of a multidisciplinary team, the UCP will play a key role in enhancing access to same-day care and reducing pressure on GPs.
Clinical Care

Conduct face-to-face and telephone consultations for urgent and same-day patient needs.

Triage and manage patients presenting with minor illness, acute conditions, or exacerbations of chronic disease.

Perform clinical assessments, take histories, examine patients, make diagnoses, and implement treatment plans.

Administer medications, prescribe (if qualified), or refer patients appropriately in line with protocols.

Provide wound care, basic procedures, ECGs, blood pressure monitoring, and other relevant assessments.
Collaboration & Communication

Work closely with GPs, nurses, pharmacists, and administrative teams to ensure coordinated patient care.

Refer patients to community, secondary care, or urgent/emergency services when clinically appropriate.

Communicate effectively with patients, families, carers, and other healthcare providers.
Governance & Quality

Maintain accurate and contemporaneous records using the clinical system EMIS web.

Participate in audits, significant event reviews, and quality improvement initiatives.

Follow practice policies and national guidelines (e.g., NICE, local ICB protocols).

Attend clinical meetings, contribute to case discussions, and share learning.

Essential
- Registered Nurse (NMC) with valid pin or HCPC registered Paramedic
- MSc or Postgraduate diploma in Advanced Clinical Practice (or equivalent)
- Non-medical prescribing qualification
- Proven experience in primary care and urgent care
- Excellent clinical reasoning, communication and decision-making skills
- Confident management of both minor illness and complex chronic conditions
Desirable
- Minor illness/minor injury or triage training
