Job summary

The GP Connect Network are excited to offer an opportunity for a Physician Associate to join our expanding team.

The PCN offers and encourages personal and professional development with great support pathways and tailored supervision from senior clinicians. We encourage our PA's to build their own peer support network within the PCN with regular meetings and communication opportunities.

Main duties of the job

  • Take patient medical histories during face-to-face or telephone consultations
  • Carry out physical examinations
  • Order diagnostic tests and analyse results
  • Make diagnoses and clinical assessments
  • Develop and deliver appropriate treatment and care management plans in consultation with patients, their relatives and carers
  • Organise further investigations, treatment and referral to a doctor / other healthcare professionals where appropriate
  • Provide patients with advice and counselling on health promotion and disease prevention
  • Maintain accurate clinical records
  • Complete paperwork and other administrative duties, including writing reports, letters and other correspondence

About us

GP Connect PCN consists of 8 GP Practices covering approx. 61000 patients. Our network operates with in the city of Coventry.

The successful candidate will join a GP Connect PCN Team and will be supported by a Senior Care Coordinator and Regular development sessions, teaching, training and opportunities to upskill will be provided.

Job description

Job responsibilities

The main responsibilities of the role include:

  • Providing direct clinical care for patients including clinical assessments, diagnosis and treatment under the supervision of a GP;
  • Interviewing patients, taking medical histories, performing physical examinations, analysing, diagnosing and explain medical problems;
  • Working with the wider practice clinical team to deliver services to meet the needs of the patients, which may include home visits;
  • Consulting and referring patients to physicians, medical specialists and other health professionals as indicated;
  • Recommending and explaining appropriate diagnostic tests and treatments;
  • Requesting and interpreting results of laboratory investigations when necessary;
  • Performing specialised diagnostic physical exams and treatment procedures;
  • Instructing and educating patients in preventative healthcare;
  • Conducting telephone consultations, which may involve discussing the result and implications of laboratory investigations with patients as well as dealing appropriately with routine and emergency complaints;
  • Fully documenting all aspects of patient care and completing all required paperwork for legal and administrative purposes.

  • Patient Management Planning
  • Minor Illness, Referrals, Prescribing guidelines
  • Working in a primary care or Hospital or A & E environment
  • Experience of using the EMIS, SystmOne clinical system
  • Experience of managing chronic disease clinics

  • Excellent communication and interpersonal skills (Written and Oral)
  • IT skills to ideally include microsoft office software and, or experience of using a clinical or similar system
  • Time management, prioritisation and the ability to work to deadlines
  • Willingness to obtain necessary training and participate in ongoing development eg venepuncture, ear syringing, chronic disease management
  • Ability to motivate patients to improve their health
