Job summary

The Advanced Practitioner - Clinical Pharmacist plays a vital role in improving patient care by providing expert medication management, supporting clinical decision-making, developing the service and collaborating with other healthcare professionals to enhance the quality of care within a Primary Care Network.

Focusing on medication safety and optimizing therapeutic outcomes, the role is instrumental in supporting the PCN goals of improving patient care by developing patient-centred care pathways, reducing hospital admissions, and enhancing overall healthcare delivery within the community.

Our team of 12 Clinical Pharmacists and 6 Pharmacy Technicians is growing and Rugby Health PCN is looking for a second Advanced Practitioner - Clinical Pharmacist. With at least 2 years PCN experience, and holding an Independent Prescriber qualification, the postholder will work collaboratively with the current AP -CP Lead as part of the wider Clinical Pharmacy Team outlined above.

Main duties of the job

As one of two practitioners you will have responsibility for half the team, sharing line management, clinical leadership and supervision, and mentoring responsibilities. You will help to guide and develop your team and focus on developing the overall service, driving clinical excellence and supporting safe, effective medicine use across our 12 practices.

Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team;

Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action;

Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice;

Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person; and

Complete the relevant training in order to provide multi-professional AP clinical practice and CPD supervision across all four pillars (with relevant training) to other roles within primary care, for example first contact practitioners and the personalised care roles. Team Management

Lead and line manage your team located across the practices within the Rugby Health PCN.

Provide mentorship and guidance to your team to support each of them to develop in their roles.

Operationally oversee the PCN Clinical Pharmacy service provided through the Clinical Pharmacists.

Manage and monitor performance of Rugby Health Clinical Pharmacists.

Provide support to the Senior Pharmacy Technician and team where required.

Work closely with Advanced Practitioner- CP Lead to manage and develop the service and the overall team, providing cover for leave as needed.

These duties are in addition to those of a PCN Clinical Pharmacist.

  • GPHC Registration as a Clinical Pharmacist
  • Masters degree in pharmacy (MPharm) or equivalent
  • Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience
  • Completed CPPE pathway
  • Independent prescriber
  • Enrol on or have completed the CP Advanced
  • Practice Programme/ Portfolio. Completion is a
  • requirement of the role.
