The Cancer Project Nurse will provide clinical expertise and professional advice to support the delivery of the Shropshire,Telford and WrekinTargeted Cancer Case Finding Programme within Shrewsbury PCN.
The post holder will work 2 sessions per week alongside the Cancer Care Coordinator, who leads on administration and patient coordination, and will provide clinical oversight, guidance, and decision-making support across the three targeted case finding pathways:
The role focuses on ensuring patients are clinically reviewed appropriately, investigations are interpreted correctly, and escalation/referral decisions are made safely and consistently, in line with the agreed specification.
Main duties of the job
Key Responsibilities
Provide senior clinical input to support targeted cancer case finding across the PCN. Review identified patient records and advise on appropriate investigations, referrals, and safetynetting. Offer clinical guidance to the Cancer Care Coordinator and liaise with the Clinical Director for complex cases.
Support delivery of targeted case finding programmes, including: Pancreatic cancer review new-onset diabetes with weight loss and advise on investigation or referral. Prostate cancer support PSA pathways, results interpretation, and onward referral. Thrombocytosis guide clinical assessment and investigations to identify possible malignancy.
Ensure all actions follow national and local guidance, including urgent suspected cancer pathways.
Provide patientfocused clinical input when needed, helping explain investigations, results, and next steps while ensuring safe and clear safetynetting advice.
Support accurate clinical documentation and validate clinical elements of reports submitted to the ICB. Identify learning and quality improvement opportunities and follow PCN governance and safeguarding standards.
Work collaboratively with practices, PCN staff, and the wider MDT. Share feedback to improve pathways and help develop consistent PCN-wide approaches to targeted case finding.

- Strong clinical knowledge and ability to assess, interpret, and advise on investigations and symptoms.

- Confident clinical decision-maker with the ability to recognise red flags and escalate appropriately.
- Excellent communication skills, with the ability to advise and support both patients and colleagues.
- Experience of working collaboratively within multidisciplinary teams.
- Good understanding of clinical systems and patient record documentation.
