Job summary

The Clinical Services Care Coordinator plays a key role in supporting Shrewsbury Primary Care Network (PCN) by coordinating clinical services and ensuring smooth day-to-day operations. The post-holder provides vital administrative and organisational support to the Home Visiting Service (HVS) and other PCN clinical teams, helping deliver efficient, patient-focused care across GP practices. This role requires excellent communication, strong organisational skills, and the ability to work collaboratively with clinicians, practices and patients.

Main duties of the job

The post-holder is responsible for coordinating rotas, route planning and service administration. Duties include liaising with GP practices, supporting Paramedics and other clinicians with scheduling, equipment and annual leave requests ensuing compliance with service requirements. The role also involves arranging meetings, handling audits and providing clear communication between GP practices, patients and the PCN teams.

Job responsibilities

Coordinating the Paramedic Home Visiting Service

  • To provide coordination and administrative support to the Home Visiting Service Paramedics and advanced clinical practitioners (ACPs).
  • To proactively manage the daily appointment structure, including route planning to keep travel distance and mileage costs down, alerting Paramedics to patients within their locality to ensure a seamless and efficient workflow.
  • Reporting to GP Practices following visits, ensuring any urgent messages to or from the Paramedics are passed to the appropriate duty doctors at each practice.
  • Complete appropriate monthly audits to ensure appointments are shared fairly across the GP Practices.
  • Liaise, involve and communicate activities to the appropriate GP Practices to ensure engagement of all parties.
  • Report and communicate activity on a regular basis to PCN Clinical Director and PCN Senior Leadership Team to ensure our on-going compliance with the Network Directed Enhanced Service (DES) requirements.
  • Manage and support the Paramedics with equipment issues/stock control.
  • Ensure annual leave requests are dealt with promptly and appropriately, making sure to maintain sufficient clinical capacity for the Home Visiting Service to run effectively.
  • Be responsible for arranging, attending and minuting Home Visiting Service meetings.
  • Proactively prepare agendas prior to the meetings, ensuring all relevant clinicians are present, and keeping a log of all outstanding actions.
  • To record patient interventions where necessary on the GP Practice medical system (EMIS).
  • To work collaboratively with other teams and services to maintain an effective and efficient service.
  • To offer appropriate support and guidance to patients and their families/carers.
  • Support with arranging training sessions and workshops for the Home Visiting Service Paramedics.

Coordinating other clinical teams

  • Provide administrative support to other PCN clinical teams, including the Mental Health Practitioners, First Contact Practitioners and Social Prescribers, coordinating their daily appointment rotas to ensure annual leave and training requirements are reflected in the appointment book.
  • Ensure annual leave requests are dealt with promptly and appropriately, remaining mindful of clinical capacity and adequate provision of services across the PCN.
  • To flag any issues clinical staff raise with the PCN Senior Leadership Team, ensuring appropriate workload is delegated to GP Practices.
  • To support with clinical observation and support via the PCN Clinical Director, arranging regular service review meeting as appropriate.
  • Supporting with rearranging patient appointments in times of staff sickness.
  • To ensure adequate resources are available for clinical teams including clinical rooms across GP Practices, and facilitating alterations in rotas when requirements change.
