Job summary

Wanting a new challenge? Our practice is looking for an enthusiastic, conscientious and passionate Care Co-Ordinator to work as part of our Community Support Team.

This is an exciting opportunity to work in a friendly, supportive, forward thinking training practice, working from a modern purpose-built premises delivering high quality health care.

Main duties of the job

Help to tackle inequalities in health for our frail, elderly patients. Working with social services and local voluntary services to help patients remain as independent as possible in the environment of their choice.

About us

Working with a small team of staff (Community Matron, Care Home Nurse and Care Co-Ordinator), but also as part of a large primary health care team.

Qualifications

Essential

  • NVQ Level 3
  • Long-term conditions general training
  • Welfare Rights basic training or willing to learn
  • Training in Motivational Interviewing
  • Knowledge of community based primary care issues.
  • Knowledge of primary care networks and collaborative ways of working.
  • Knowledge of social care structures
  • Knowledge and understanding of community services
  • Adult Safeguarding training
  • Knowledge of systmone

Desirable

  • Knowledge of medical terminology
  • Knowledge of social services structures
  • Vulnerable adults awareness
  • Experience of Care of the Elderly and pharmacy
  • Training in continuing care criteria

Skills

Essential

  • Excellent written and verbal communication skills and interpersonal skills
  • Computer literate
  • Able to manage own workload recognising priorities
  • Team player and able to work in collaboration with others
  • Excellent motivational ,influencing and negotiating skills
