Job summary

We are seeking to build our PCN pharmacy team and employ a self-motivated pharmacy technician to join us to help the PCN support our community and improve the health and wellbeing of our patients.

Clinical Pharmacy in General Practice is part of an exciting programme of transformation to develop a new model of care which addresses our ambition to deliver coordinated person-centred care across the district.

The Clinical Pharmacy in General Practice model is supported by the direction of the Five Year Forward View and GP Forward View where there is a need to better utilise the role of pharmacy within primary care and to pro-actively help patients stay safe and out of hospital as well as helping to reduce the demands on general practice.

Pharmacy technicians are different from clinical pharmacists as they are not able to prescribe or make clinical decisions, instead working under supervision to ensure effective and efficient use of medicines.

Pharmacy technicians can therefore play an important role by complementing clinical pharmacists, community pharmacists and other members of the PCN multi-disciplinary team.

The post holder will have to be a registered pharmacy technician with the General Pharmaceutical Council (GPhC) with the appropriate qualifications recognised by the GPhC.

All Pharmacy Technicians must have completed or be enrolled in an approved training pathway (e.g. Primary care pharmacy educational pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH)).

Main duties of the job

1. Undertake patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients.

2. Carry out medicines optimisation tasks including effective medicine administration (e.g. checking inhaler technique), supporting medication reviews and medicines reconciliation. Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively.

3. To reconcile medicines from clinical letters and discharge summaries from secondary care settings, support medication reviews and medicines reconciliation fornew care home patients and synchronising medicines for patient transfers between care settings, linking with local community pharmacies

4. Support the Clinical Pharmacist in Structured Medication Reviews (SMR) i.e. organise necessary monitoring tests prior to SMR

5. To liaise with community pharmacy to ensure safe and timely supply of medication.

6. Provide expertise to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities.

7. Manage shared care protocols and liaise with Clinical Pharmacists for more complex patients.

8. Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing locally.

NASAR PCN comprises of 5 Practices in Washwood Heath Birmingham. The practices have a combined geographic area made up of over 30,000 patients. There is a wide socio-economic and demographic served within this area which will provide an interesting and varied case mix. You will be joining an enthusiastic team of clinicians and administrators. You will be well supported with ongoing professional development (training support, CPD, and peer support). The role will contribute to the improving quality of care of our patients across the network of practices.

The PCN Pharmacy Technician will only need to work at 1 Practice-Naseby Medical Centre.

Naseby Medical Centre is excited to increase its growing number of clinical staff which currently includes 3 full time GPs, 2 PCN Clinical Pharmacists and 1 PCN First Contact Physiotherapist

The PCN Pharmacy Technician will be under the supervision of either a GP or Clinical Prescribing Pharmacist depending on the availability and working day of the clinician.

Helping towards QOF (ensuring appointments are made for long term conditions in line with birth month etc)

Medication review monitoring etc ensuring urate is done for Allopurinol, ECG for QOF, Prolactin for Antipsychotics etc

Updating Ophthalmology meds/drops

Appointments 5-6 a week, majority for new patients but also some for reductions of meds or titrations.

Setting up eRD (weekly, monthly etc)

Medicines Optimisation projects in line with local drives and formulary updates (deprescribing OTC meds, checking iron/folate on repeats which levels are now normal and no longer needed.

Checking formulary items/updates making swaps

Issuing medication alternatives

Helping change medication to liquids if swallowing difficulties

Clinical Responsibilities:

Undertake patient appointments; supporting roles to ensure effective medicines use, through shared decision-making conversations with patients. Carrying out medicines optimisation including effective medicine administration, supporting medication reviews and reconciliation.

Carry out medicines optimisation tasks including effective medicine administration (e.g. checking inhaler technique), supporting medication reviews, and medicines reconciliation.

Support quality improvement measures and contribute to the Quality Outcomes Framework and Enhanced Services.

Technical and Administrative Responsibilities:

Work with the Practice / PCN multi-disciplinary teams to ensure efficient medicine optimisation.

Supervise practice reception teams in sorting and streaming general prescription requests.

Provide leadership for medicines optimisations systems.

Provide training and support on the legal, safe and secure handling of medicines, including the implementation of the Electronic Prescription Service (EPS).

Develop relationships with other pharmacy technicians.

Support the pharmacy team across health and social care including primary care, community pharmacy, secondary care and mental health.
