Job summary

Wimborne and Ferndown PCN are looking to recruit a Social Prescriber to join our friendly team working across our 5 practices.

Main duties of the job

To provide holistic patient assessments through motivational interviewing and guided conversations. To co-design a social prescription with the patient to improve the health and well-being outcomes for individuals with a longer-term outcome of reducing the number of clinical/ medical interventions and to promote lifestyle change to prevent ill-health using patient- led behavioural change techniques.

About us

Wimborne & Ferndown PCN provides services to 42,000 patients across 5 Practices (Quarter Jack Surgery, Walford Mill Medical Centre, Old Dispensary, Pennys Hill Practice, Orchid House Surgery). We have adopted new roles within the PCN to support and broaden our primary care team which allows us to provide better care for our patients.

The PCN is headed by our committed Clinical Directors and a Network Business Manager. We also have a Care Coordinator team who support the overall network team and services.

In addition to the PCN, our Practices are also part of Castleman Healthcare, a non-profit company owned by 13 GP Practices in North Poole and East Dorset (3 PCNs). Castleman Healthcare offer additional services to our Patients through separate contracting arrangements, provide business and practical support to the PCNs and support with representing the PCNs voice within the wider healthcare system.

Job description

Job responsibilities

To undertake holistic assessments and co-design Health and Well-being Plans with individual patients, identifying support needs to ensure maximum engagement in improving health and well-being.

To work with patients, using behaviour techniques such as motivational interviewing and guided conversations to support them to identify, set and meet achievable goals to improve their physical and emotional wellbeing.

To provide patients with continuity and a coordinated experience of care and remaining the point of contact throughout the individuals social prescription.

To work with patients in GP surgeries to signpost them to local services / organisations which the patient themselves feel would be of benefit to reach their goals. These services may include, befriending, weight management, mental health support, smoking cessation, local social groups, leisure centres and volunteering etc. to name but a few.

To follow up with patients, update their records and support them on an ongoing basis.

To make and maintain contact with local health and wellbeing service providers and build a network and knowledge of referral routes to and from service providers.

To ensure information on sources of voluntary and community support is up to date at all times to enable effective and accurate signposting and linking of individuals with services.

To undertake regular training to extend and update own knowledge of health and wellbeing issues.

To establish and maintain effective liaison with stakeholders including health, voluntary, social and education resources, attending relevant meetings as necessary.

To train and develop GPs and primary care health teams knowledge on how to identify patients suitable for social prescribing service referral and to keep the service at the forefront of clinicians minds by means of being present at MDTs, social media, regular updates, practice meetings etc. Work closely with the Practice Managers to ensure their understanding of the role and enable them to disseminate accurate information to their teams.

Maintain a SystmOne template to ensure accurate and detailed clinical data is entered into the patient record in a standardised way. To keep records of your work and adhere to confidentiality, information sharing protocols and provide monitoring information as required.

Provide comprehensive outcome focused reports detailing the progress of the service using information from the template above where required.

To ensure you have an understanding (appropriate to your role) of, and comply with procedures for promoting and safeguarding the welfare of children and vulnerable adults.

  • A health, social care, counselling or other relevant professional or academic qualification or background, including motivational interviewing or guided conversations training.
  • Excellent holistic assessment as well as consultation skills and experience of providing empowering support to adults in a planned and structured way to improve health, recovery and well-being outcomes.
  • Experience of training, supervising, motivating, mentoring and developing individuals to ensure a high quality service that meets the need and promotes positive outcomes for individual patients.
  • Proven skills using community resources appropriate for a range of different patients from various communities and backgrounds.
  • Excellent record keeping skills.
  • A proven understanding of safeguarding of children and vulnerable adults and ability to implement relevant policies and procedures.
