Youll play a key role in delivering highquality patient care by managing all aspects of medication, including repeat prescriptions, medication reviews, and drug monitoring. Working closely within a multidisciplinary clinical team, youll combine strong clinical judgement with patientfacing care to ensure safe, effective, and timely prescribing. This role offers autonomy, a manageable patient list, and the opportunity to contribute to service development within a collaborative primary care environment.
Main duties of the job
Key responsibilities
- Manage all aspects of patients medication needs, including repeat prescriptions, medication reviews, and drug monitoring.
- Provide safe, accurate, and timely prescribing and medication management.
- Maintain a personal patient list and deliver highquality clinical care.
- Collaborate closely with the clinical team to support integrated patient care.
- Work efficiently with the Pharmacist Dispenser to streamline repeat prescribing workflows.
- Contribute to shared learning within the wider pharmacist team and support continuous service improvement.
- Demonstrate strong communication, clinical judgement, and organisational skills to balance speed and accuracy in a busy primary care environment.
- Uphold professional standards, including maintaining valid GPhC registration and applying robust clinical knowledge in daily practice.
