PCN Paramedic / Paramedic Advanced Practitioner

Job responsibilities

The Paramedic will play an important role in supporting the PCN and member practices with managing urgent and unscheduled care. Your role will be to provide clinical assessment by remote (telephone or video) and face to face appointments, to diagnose, treat, refer or signpost patients/service users who attend the surgery with undifferentiated or undiagnosed conditions relating to acute illnesses or minor injuries. You will use advanced clinical skills to provide education to service users, promoting self-care and empowering them to make informed choices about their treatment and provide a Specialist Paramedic resource for the Practice Team and service users. The post holder will work with the other members of the primary care team to support the management and decision making about care and service provision for individual patients.

Duties and responsibilities

Work with our GPs and other primary care professionals in order to identify and manage a caseload of patients with specific needs, flagging any concerns to the relevant GP and referring to other health professionals as appropriate.

Act as an expert practitioner, demonstrating clinical competence and a sound knowledge base.

Make direct referrals to primary, secondary and social services within locally agreed pathways, guidance and protocols.

Provide clinical advice to Reception staff where required.

Ensure that patients receive high quality clinical care, delivered in a timely manner.

Ensure complete and accurate documentation of each and every patient contact.

Work as an independent practitioner and as part of the multidisciplinary and multi-agency teams, in order to ensure patients needs are met.

Ensure that professional standards are maintained and within the guidance of the Department of Health and the Nursing and Midwifery Council (NMC)/Health and Care Professions Council (HCPC) and the College of Paramedics (COP)

Qualifications

Essential

• Qualified Paramedic Current registration on the HCPC register Post registration experience of which several years in primary care, A&E unscheduled care or related field.

Desirable

• Minor Illness/injuries or Clinical skills qualification Completion of or working towards an independent prescriber qualification Completion of or working towards ACP

Experience

Essential

• Experience of working to protocols or guidelines Ability to work autonomously, prioritise own workload, work under pressure and maintain high quality care Knowledge of unscheduled care practice. Excellent clinical skills. Knowledge of clinical governance Understanding of, and commitment to, equality, diversity and inclusion Knowledge of IT systems to include the ability to use word processing skills, emails and internet An appreciation of the nature of GPs and general practices Ability to build rapport and gain trust with patients Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences

Desirable

• Experience in a primary care setting willingness to undergo additional training, education and mentoring to develop and maintain clinical skills. Flexibility to work across different sites. Ability to undertake training during office and own hours. Experience of using GP clinical systems e.g. EMIS Knowledge and experience of undertaking audits
