Job summary

An exciting opportunity has arisen for a Paramedic to join Dartford Model Primary Care Network as part of their growing multi-disciplinary team. We pride ourselves on providing excellent patient centred care and wish to expand our paramedic team to deliver high quality paramedic services across our network.

Working in our established PCN clinical team, alongside our existing paramedic and pharmacists, you will focus on supporting the practices with managing urgent and unscheduled care. Your role will be to provide clinical assessment by remote (telephone or video) and face to face appointments, to diagnose, treat, refer or signpost patients/service users who attend the surgery with undifferentiated or undiagnosed conditions relating to acute illnesses or minor injuries. To use advanced clinical skills to provide education to service users, promoting self-care and empowering them to make informed choices about their treatment. This may include working until 8pm weekday evenings or 9am-5pm on Saturdays as part of a rota to provide care to patients across the PCN from 8am-8pm Monday to Friday, and from 9am-5pm on Saturdays.

Main duties of the job

The post holder must be a registered Paramedic with post graduate qualifications relevant to the position. Experience in General Practice and independent prescribing training would be an advantage but is not essential. More important is a positive attitude and expertise in engaging people; and the ability to work with a wide range of people to ensure all patients receive the best possible care and service.

You will be engaged with the Roadmap to Practice pathway to support your role as a First Contact Practitioner. We would be interested in applications from Advanced Practitioners. The salary for this role will reflect the Skills and Experience of the applicant

Dartford MODEL PCN covers around 32,000 patients registered with Wellcome Practice, Dr Shimmins and partners (Dartford East) and Lowfield Medical Centre. Primary Care Networks (PCN) are groups of GP practices working closely together with other primary and community care staff and healthcare organisations to provide integrated services to their local populations. They work collectively to understand the health needs of their population and to deliver improvements to the quality of care in provided in primary care, care outside of hospital and across the PCNs.

Job responsibilities

The Paramedic will play an important role in supporting the PCN and member practices with managing urgent and unscheduled care. Your role will be to provide clinical assessment by remote (telephone or video) and face to face appointments, to diagnose, treat, refer or signpost patients/service users who attend the surgery with undifferentiated or undiagnosed conditions relating to acute illnesses or minor injuries. You will use advanced clinical skills to provide education to service users, promoting self-care and empowering them to make informed choices about their treatment and provide a Specialist Paramedic resource for the Practice Team and service users. The post holder will work with the other members of the primary care team to support the management and decision making about care and service provision for individual patients.

Duties and responsibilities

Work with our GPs and other primary care professionals in order to identify and manage a caseload of patients with specific needs, flagging any concerns to the relevant GP and referring to other health professionals as appropriate.

Act as an expert practitioner, demonstrating clinical competence and a sound knowledge base.

Make direct referrals to primary, secondary and social services within locally agreed pathways, guidance and protocols.

Provide clinical advice to Reception staff where required.

Ensure that patients receive high quality clinical care, delivered in a timely manner.

Ensure complete and accurate documentation of each and every patient contact.

Work as an independent practitioner and as part of the multidisciplinary and multi-agency teams, in order to ensure patients needs are met.

Ensure that professional standards are maintained and within the guidance of the Department of Health and the Nursing and Midwifery Council (NMC)/Health and Care Professions Council (HCPC) and the College of Paramedics (COP)
