Job summary
We are Hanley, Bucknall & Bentilee PCN.
To support our PCN practices in the delivery of the Facilitation of
Admission Avoidance (FAAS) service, delivering care within their scope of
practice to the entitled patient population. The care coordinator will deliver
and assist clinical staff in preventative care, health promotion and patient
education.
The successful candidate will be required to perform home visits but
also from any of the premises where the practice or PCNs patients are being
managed.
Main duties of the job
There may be on occasion, a requirement to carry out other tasks; this will be
dependent upon factors such as workload and staffing levels:
Visit patients in their
homes to conduct holistic needs assessments
Conduct holistic needs
assessments in the surgeries
Hand out patient
satisfaction surveys after each consultation & assist with
completion of these if needed
Complete referrals for
patients
Liaising with surgery staff
regarding patient concerns
Carry out baseline
observations such as pulse oximetry, blood pressure, temperature, pulse
rate, recording findings accurately
Facilitate routine & 24-hour BP monitoring, advising patients accordingly
Carry out BMI checks
To possibly undertake venepuncture
Ensure specimens are
recorded & ready for onward transportation
Ensure clinical waste is
removed from clinical areas & sharps bins replaced in accordance with
the practice IPC policy
Deliver opportunistic health
promotion where appropriate
Ensuring samples are stored in appropriate containers
Accurate labelling of all samples to confirm patient identity
Checking patient identification against GP/nurse request & confirming
with patient whenever possible
Ensuring that anomalies are reported to a senior member of staff
Notifying the GP/nurse of any significant patient issues
Liaising with and assisting members of the administrative team

We are a PCN made up of 4 surgeries, in Hanley, Bucknall & Bentilee PCN. We have 33,000 patients.
Our PCN undertakes the holistic needs assessments for the FAAS scheme, as a PCN.
