We are seeking a dedicated and compassionate Practice Nurse to join our healthcare team. Candidate must hold a level 6 diploma or equivalent and have at least two years experience in general practice and chronic disease management.

About us

Portland Road Practice is a multidisciplinary team that consists of; 6 GPs, 1 GP Registrar, 2 FY2's, 1 Nurse, 2 HCAs and 2 Pharmacists. All supported by our amazing admin team. We serve over 9000 patients in the Notting Hill, Kensington and Shepherds Bush areas; the practice clientele includes a cross-section of socio-economic and ethnic groups reflecting the vibrant cosmopolitan mix of the area.

The ideal candidate will play a key role in providing high-quality, patient-centered care to individuals of all ages, promoting health and wellbeing within the community. This role will involve performing health assessments and screenings, administering vaccinations and immunisations, wound care, and dressing changes, and providing chronic disease management. The nurse will also offer health education, advice on lifestyle changes, weight management, and mental health. The position requires taking and recording patient medical histories, administering prescribed medications, and ensuring follow-up care in collaboration with GPs. Additionally, the nurse will be responsible for maintaining accurate patient records, educating patients on medication usage, and staying current with medical practices. Qualifications include being an NMC registered nurse, with experience in a general practice setting preferred. Strong communication skills, organisational ability, and a patient-focused approach are essential, as is the ability to maintain confidentiality. Desirable skills include experience in chronic disease management and preventive healthcare.
