Job summary

You will be an Advanced Clinical/Nurse Practitioner and Independent Prescriber and able to manage patients autonomously. These will be with a minor illness focus across the age range.

You will undertake a variety of duties including face to face consultations, telephone consultations, and home visits. This will be for patients across the age range.

You will be responsible for providing NHS medical services to patients on the Practice list. This will have a minor illness focus but can be shaped based on particular clinical knowledge and experience.

Main duties of the job

The AP will work to deliver high quality clinical care. The split is flexible and can be agreed to meet the preference of the AP and the needs of the PCN. The role will involve assessment, diagnosis and treatment planning of patients across the age range with support from advanced practitioners and GPs.

Herne Bay PCN is committed to the education and development of its staff, and APs will be supported to accredit with Health Education England when this becomes available for their profession.

The position is open to nurses or paramedics meeting the Advanced Practice Four Pillars in Primary Care Medicine.

Job description

Job responsibilities

The following are the core responsibilities of the paramedic practitioner. There may be on occasion a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels. The paramedic practitioner will work within their scope of clinical practice and:

a. Identify, signpost or refer patients at risk of developing long-term conditions, preventing adverse effects on the patients health

b. Provide routine care to patients as required in accordance with clinically based evidence, NICE and the NSF

c. Examine, assess and diagnose patients and provide clinical care/management as required

d. Triage patients and provide the necessary treatment during home visits

e. Collect pathology specimens as required

f. Maintain accurate clinical records in line with extant legislation

g. Ensure that read codes are used effectively

h. Ensure continuity of care, arranging follow-up consultations or reviews as necessary

i. Review the effectiveness of the treatment provided, making changes where necessary to improve patient outcomes

j. Chaperone patients where necessary

k. Prioritise health issues and intervene appropriately

l. Act as the lead clinician when dealing with clinical emergencies

m. Recognise, assess and refer patients presenting with mental health needs

n. Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)

o. Liaise with external services/agencies to ensure that the patient is supported appropriately (vulnerable patients, etc.)

p. Ensure that they adhere to the relevant patient group directives and local clinical pathways at all times

q. Support the clinical team with all safeguarding matters, in accordance with local and national policies

r. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

s. Deliver opportunistic health promotion where appropriate
