Job summary

We are looking to recruit an enthusiastic and self-motivated individual on a fixed term basis to develop their skills in this innovative role as a Pharmacy Medication Reconciliation Clerk in General Practice.

You will be part of an innovative practice team that integrates pharmacy skills as part of co-ordinated care to improve patient outcomes and safety whilst also supporting the Medicines Optimisation Model. The existing team has already helped drive significant improvements in care provision and workload shifting with integration of skill mix within the wider healthcare team.

Main duties of the job

To work within protocols to update medication for patients in a timely manner (letters, clinical plans agreed with clinicians, specific plan agreed with pharmacy team).

To communicate with patients in respect to specific patient plans.

To deal with prescription queries in a timely manner to maintain the prescriptions turnaround time

To support patients in the standard ordering system and/or identify specific patient's groups requiring extra support with the ordering system

To liaise with pharmacies in order to enable patients to obtain their medication safely and in a timely manner

To act as point of contact for prescription administration team, to audit the safety and effectiveness of the current processes, suggest improvements and implement them

To support the administration staff with medicines reconciliation in accordance with practice protocols and to update patients medical records/ prescribing accordingly

To contribute to the review and development of repeat prescribing protocols / standard operating procedures to improve the efficiency and effectiveness of repeat prescribing systems
