Job summary
Radcliffe Medical Practice is looking to recruit an experienced practice nurse to join our clinical team, to deliver essential nursing care and supporting the health and wellbeing our of patients. We will also consider applications from registered nurses who are looking to train to be a practice nurse. You will join a team made of GPs, Advanced Nurse Practitioners, Paramedic practitioners Clinical Pharmacists, Social Prescribers, and Physiotherapists.
This is a part time role of 30 hours worked over 3 days. We are a training practice and can support your ongoing development.
Main duties of the job
Have knowledge of EMIS web
Provide a full range of general practice nursing care including childhood immunisations, vaccinations, smear taking, chronic disease reviews and family planning.
Support health promotion and wellbeing including national programmes such as NHS Health Checks, cervical screening, and lifestyle advice.
Support the management of Long term conditions such as asthma, Diabetes and hypertension.
Provide additional treatment room activities such as ECGs, B12 injections
Work with our practice and nursing team on the overall development of the practice and protocols relevant to the role
Provide training for staff on infection control.

- To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
- To develop, implement and embed health promotion and wellbeing programmes
- To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing in line with the Quality Outcomes Framework
- To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health
- To provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF
- To manage acute and chronic conditions
- To coordinate the provision of travel medicine services
- To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
- To maintain accurate clinical records in conjunction with extant legislation
- To ensure read codes are used effectively by all members of the nursing team
- To ensure the appropriate maintenance and use of chronic disease registers
- To develop, implement, embed and participate in well woman clinics
- To develop, implement, embed and participate in well man clinics
- To prioritise health issues and intervene appropriately
- To support the team in dealing with clinical emergencies
- To recognise, assess and refer patients presenting with mental health needs
- To implement vaccination programmes for adults and children
- To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
