Job summary
The Advanced Paramedic Practitioner will work autonomously within the GP Practice
and the community, using their enhanced clinical assessment and treatment
skills, to provide first point of contact for patients presenting with
undifferentiated, undiagnosed problems such as minor illness or injury,
abdominal pains, chest pains and headaches and holistic needs assessment of
patients with long term conditions. The Paramedic Practitioner will have a key
role in supporting delivery of the new Network Contract DES Service
specifications.
The successful candidate will have excellent communication skills and
will work closely with our GPs, PCN clinical pharmacists, nurses, physician
associates, administrative and reception teams. You will have good
time-management skills and be able to prioritise tasks and work independently.

Radcliffe Medical practice is looking for motivated, enthusiastic experienced Advanced Paramedic Practitioner to join our expanding team. This is an opportunity to be involved in developing new ways of working in primary care with our friendly, forward-thinking practice teams within Bury PCN. RMP is a fantastic place to work with its diverse, varied population, and friendly, supportive practice Team. We have a list size of around 10,600 patients. Our clinical Team consists of two GP Partners, six salaried GPs, 1 advanced nurse practitioner, 1 paramedic practitioner, 3 practice nurses, and a HCA. We also host the following PCN staff, 2 GPAs, a physician associate, and a pharmacist.
We are a training practice and provide training for medical students, FY2 and ST3 s. We can offer support and training in the role to the right candidate.

The Paramedic Practitioner will:
- Assess and triage patients, including same day triage, and as appropriate provide definitive treatment.
- Undertake home visits for acute illness or chronic disease management.
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
- Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs and those at risk of developing a long term condition.
- Advise patients on general healthcare and promote self-management.
- Perform specialist health checks and perform investigatory procedures as required.
- Support the delivery of anticipatory care plans and lead certain community services (e.g. monitoring blood pressure).
- Provide an alternative model to urgent and same day GP home visit for the practice and undertake clinical audits.
