Job Summary:
The post holder will:
Support the management of chronic diseases and QOF
Support the delivery of medicines safety agenda
Support the duty doctor with day-to-day medication queries
Support /lead in quality improvement projects involving medicines
Support medicines optimisation across the interface
Carry out medication review and medication related queries to ensure patients get the best out of their medications.
Promoting safe and evidence based prescribing within the PCN
Support the delivery of the pharmacy and medicines optimisation elements of the network service specifications within the Network Contract DES
Support the training of other staff members
Key working relationships

CCG Pharmacists and Technicians
Community Pharmacy
Care Home and Hub pharmacy staff
Any other healthcare provider as necessary

MAIN DUTIES AND RESPONSIBILITIES OF THE POST
OVERVIEW and SPECIFIC RESPONSIBILITIES
Clinical Pharmacy Service (CPS) within WISE 2 Primary Care Network. The post holder will be predominantly based as Sunny Meed Surgery; this may also include branch sites Greenfield and Goldsworth Park.
The clinical pharmacy service will be primarily patient facing and clinical in nature focused on improving medicines optimisation and safety working as part of a multidisciplinary team. Main elements include:
- Management of chronic diseases such as asthma, COPD, diabetes, heart failure, hypertension. Working towards Quality Outcome Framework.
- Contribute to the Greener NHS programme, promoting greener use of medications.
- Working along side the duty doctor to manage on the day queries about medications, ensuring safe and effective prescribing, following local and national guidelines.
- Undertaking structured medication review and optimisation to tackle over-medication of patients, in particular elements will include:
- Inappropriate use of antibiotics
- Withdrawing medicines no longer needed
- Structured medication reviews to support patients to take their medicines to get the best from them, reducing waste and promoting self-care.
- Dedicated focus on priority groups:
- High risk and addictive medications
- Polypharmacy
- Care Home residents
- Frail elderly
- Patients with complex needs taking large numbers of different medicines.
- Management of medicines after discharge from hospital, reconcile medicines following discharge from hospitals and intermediate care including identifying and rectifying unexplained changes and manages these changes without referral to a GP.
- Promoting safe and evidence-based prescribing within the PCN
- Leading on the implementation new guidelines and recommendations on medicines use.
- Manage the clinical aspects of shared care protocols, clinical research with medicines and, liaison with specialist pharmacists
- Supporting the practice to meet CQC standards around prescribing and medicines.
- Acting on medicines safety alerts and managing medication shortages.
- Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individualised patient reviews. Put in place changes to reduce the prescribing of these medicines to high risk patient groups.
- Ensure that patients are referred to the appropriate healthcare professional for the appropriate period of time e.g. acute conditions, minor ailments, long term condition reviews etc.
- Supporting the delivery of the pharmacy and medicines optimisation elements of the network service specifications within the Network Contract DES with a particular focus on the structured medications review and optimisation specification and the enhanced health in care homes specification.
- Undertake and complete the Primary care Pharmacy Education Programme and independent prescribing if required
- Support the delivery of the Medicines Safety agenda and medicines related QoF quality improvements
- Collaborate with pharmacy colleagues across the different sectors to ensure medicines optimisation across the interface.
- Supporting further integration of general practice with the wide healthcare teams (including community and hospital pharmacy) to help improve patient outcomes.
- To assist in the training of pharmacy staff and other healthcare workers
- Be involved in clinical pharmacy training/teaching to pre-registration pharmacists, technicians and junior pharmacists.
- Provide pharmaceutical education and training to other healthcare staff in all areas to which you have a responsibility.
- Take part in the system wide education and training programmes for all healthcare staff.
- Provide education and training to primary care healthcare team on therapeutics and medicines optimisation.
- To follow all Health and Safety standards in compliance with the practice and PCN policies and procedures. This includes:
- Report all accidents, incidents and near misses
- Dealing with hazardous chemicals (including cytotoxic drugs) used in the department, following COSHH regulations.
Carry out the following in all areas of work to:
- Develop, review and follow all standard operating procedures (SOPs) accurately.
- Maintain accurate manual and computer records.
- Housekeeping duties
- Ensure professional and legal standards are adhered to
- To participate in specified interface project/audits, according to need that supports medicines optimisation activities. The definition of these projects will link to issues arising at PCN level and actions arising out of the medicines safety QoF QI pertaining to interface matters.
All post holders:
- Will be a member of the North West Surrey Clinical Pharmacy Network which meets bimonthly
- Will be supported by a senior clinical pharmacist and by GP Clinical supervisor
- To maintain registration, as a pharmacist, with the General Pharmaceutical Council, and to act in accordance with the code of ethics.
- The post holder agrees to participate in Continuing Professional Development (CPD) including further training and learning in order to develop the knowledge, understanding, skills, attitudes and values to become competent in the duties described. To undertake peer review and competency based assessment to ensure required standards of service are maintained.

Your hours are based on a whole time week of 37.5 hours. Hours are calculated excluding meal breaks and are to be worked in accordance with the needs of the service and as agreed with your manager. The way in which the contracted hours are worked may change in accordance with the needs of the service, following consultation
