Job summary

We are looking for a Pharmacy Technician to join our GP surgery team. In this role you will play a key part in ensuring safe, effective prescribing and high-quality patient care.

In line with practice protocols, you will support medication reviews and audits, contribute to long-term condition (LTC) management, and respond to patient medication queries. Working under the supervision of a clinical pharmacist or GP, youll provide safe and effective medicines advice, liaise closely with community pharmacies, and promote online ordering to help minimise clinical risk and reduce medicines waste.

Main duties of the job

  • Support medication reviews.

  • Work closely with prescription clerk team and clinical team

  • Assist with long-term condition management under supervision.

  • Help identify and monitor high-risk patients.

  • Reconcile medicines

  • Respond to medicines queries and signpost appropriately.

  • Manage repeat prescription requests and ensure monitoring is up to date.

Job description

Job responsibilities

  • Provide patient-facing medicines support through clinics or on-demand requests, including medication adherence reviews, polypharmacy checks, and technical support for structured medication reviews.
  • Deliver aspects of long-term condition clinics (e.g: DOAC reviews, statin counselling) within scope and under supervision or clear SOPs.
  • Support risk stratification processes by identifying high-risk patients , and escalating complex cases to the clinical pharmacist or GP as needed.
  • Review medicines linked to unplanned hospital admissions and contribute to reducing avoidable harm through targeted audits and interventions.
  • Reconcile medicines following hospital discharge, including reviewing changes, ensuring continuity of supply, and collaborating with community pharmacies and patients.
  • Respond to medicines-related enquiries from practice staff, patients, and wider healthcare teams, providing clear information and escalating complex issues when appropriate.

  • Mandatory registration with the General Pharmaceutical Council as pharmacy technician
  • Demonstrates understanding of, and conforms to, relevant standards of practice
  • Follows professional & organisational policies/procedures relating to performance management

  • Foundation training in primary care
  • Skills and Knowledge

  • -Adaptable and self-motivated
  • -Meets DBS reference standards and has a clear criminal record, in line with the law on spent convictions
