i3 PCN is offering an exciting opportunity for an experienced First Contact Physiotherapist to join our dynamic multidisciplinary team. This role is essential in transforming and modernising pathways of care, facilitating effective collaboration across traditional professional boundaries.

Role Overview

Act as the first-contact physiotherapist, providing clinical expertise to make informed decisions about the best course of action for patients' care, including those with undifferentiated conditions. This involves seeing patients without prior contact with their GP to establish rapid and accurate diagnoses and management plans.

Demonstrate expertise in performing joint injections as part of the treatment plan for musculoskeletal conditions, ensuring optimal patient outcomes and pain management.

Progress and request investigations to facilitate diagnosis and treatment choices, understanding the limitations of information derived from these tests. Demonstrate knowledge of the sensitivity and specificity of diagnostic services such as x-rays and blood tests, and interpret results to aid in the management of patients.

Deliver programs that support patient self-management, focusing on behavioural change to optimise physical activity, mobility, and independence. Aim to fulfil personal goals while minimising the need for pharmacological interventions.
