Job summary

To deliver high-quality primary care services as part of the Primary Care Network (PCN),

supporting improved patient access, health outcomes, and system efficiency in line with the

Network Contract DES, with a focus on capacity and access

Main duties of the job

Clinical Duties

Provide a full range of GP services, including

Face to face consultations

Telephone and video consultations

Home visits where appropriate

Diagnose and manage acute and chronic conditions

Prescribe in accordance with local and national guidelines

Maintain accurate and contemporaneous clinical records

Support the practice using online consultation tools and Total Triage

Will include on call duties

PCN & ARRS Role-Specific Duties

Support enhanced access and same-day care provision

Contribute to:

Holistic care

QOF achievement

Structured medication reviews (where appropriate)

Care of patients with complex needs

Support delivery of PCN service specifications

This exciting and rewarding role will provide you with the opportunity to apply and develop

your clinical expertise. You will be key to the Practice in developing its holistic approach to

patient services through the development of chronic disease activity. Specifically you will

focus on clinical nurse practice and the care & programme management for Respiratory care

and Heart failure care.

The Practice Specialist Nurse will be a vital member of the Practice team providing clinical

leadership and driving clinical practice development.
